Understanding of Infertility - Must For Infertile Couple

Human reproduction is the highest and complex function of the body. In bodily development this comes last – considers as sign of puberty and again this function vanishes first as age advances and is considered as sign of increasing age. Abnormality in any organ of the body can effect reproduction adversely. Any organs abnormalities should be considered very important in treating infertility. Each childless couple must have through knowledge of reproductive physiology.

Important factors for fertility:-

  • Semen should be normal - Male Factor
  • Semen should be deposited at proper place in vagina-Coital Factor
  • Cervix should be able to transfer the semen properly forward-Cervical Factor
  • Uterus should be anatomically and functionally normal-Uterine Factor
  • Fallopian tube should be functioning normally-Tubal Factor.
  • Ovary should be secreting normal ova-Ovarian Factor
  • Pituitary and other endocrine glands  physiologically normal - Endocrine Factor
  • Mind in sound psychological state-Psychological Factor.

Here I would like to elaborate only few important factors for considerations of my patients particularly on * Psychological Factor / *Cervical Factor / *Male Factor. Other factors are mainly medico-technical so for common mass understanding of these topics may not have place here, also because of space constrain does not permit me. So excuse me for non inclusion of information’s of all factors here. However if any one is interested I will be very pleased and encouraged to give detailed information on any required topic.

Psycho Social Aspects of Infertility
Infertile Couples should always understand that following emotions, which I am describing now, are very common and if any of infertile couple is reading this write up, they will be much benefited in knowing these aspects of infertility. For all those, I request to have print out version of this article and keep within reach for reading often.

At the marriage most couple simply take it for granted that they will be able to have children. Unfortunately, this is not always the case. In fact, one in five couples trying to have a baby will experience problems in doing so. Couples are often shocked when they discover that they are infertile and commonly at beginning go through a period of disbelief. Few others rush into treatment without proper diagnosis and planning. 

Infertility is often described as a life crisis. Infertility is only medical condition that has many emotional aspects. Feelings such as anger, sadness, guilt, and anxiety are common and may affect couple’s self-esteem and self-image. Couple may find it difficult to share their feelings with family and friends, which can lead to isolation. It is important to know that these feelings are normal responses to infertility and are experienced by many couples. Ability of physician to deal with psychological problems is of paramount importance simultaneously with treatment of infertility. I have empathy for all my infertile patients and I always take care about psychological aspects first and at least make them comfortable.

After shock and feelings of denial, during fertility treatments when they do not get desired result as per expectations, couple experience anger. Guilt and blame are other common emotions. This can lead to severe depression, isolation, and self-esteem problems… Unspoken guilt and blame can make life very difficult. Couple might also find it increasingly difficult to go to work, socialize, or even get out and meet family and friends. Couples require not only time and money, but also intense emotional commitment for treatment. This depression can lead to numerous physical side effects and put a strain on relationships, work, and enjoyment of daily life.

Feelings such as worthlessness, difficulty in sleeping or sleeping too much, lack of interest in activities or relationships, change in appetite, weight loss or gain, alcohol or drug, thoughts of suicide may also be created.

In this life challenging situations infertility specialist can do lot for couple. As infertility specialist it is always my first endeavor to support in psychological aspects of couple. Absolute humane and heart felt approach is must by physician. Couple should be helped in all aspects including establishing life style and in monitory aspects also-for weaving few charges or giving concessions to encourage their psychology and positive feelings.

Each couple should view the treatment of infertility as a process journey. Remain positive but don't expect to see results right away. Couple should get Involved in Life. It is important not to lose sight of everyday activities or pursuing an activity that interests you.

Complimentary alternative therapies not only tackle infertility but aid in life style positive build up. Self hypnosis training helps not only for psychological aspects but it brings success very early.

Cervical Factor of Infertility
Around time of ovulation, because of a rising estrogen levels, the cervical mucus become thinner and thus makes it easier for sperm to enter and swim up into the uterus. After the coitus external cervical is bathe in the seminal pool.

Once sperm are ejaculated into the vagina they must swim through the cervical mucus, pass the cervix, and enter the uterus. Their final destination is the end of the fallopian tube, where fertilization occurs. Abnormalities in the cervical mucus can slow, damage, or destroy sperm.

A test known as a Post-Coital Test (PCT) (sperm-mucus interaction test) has been the traditional method for identifying cervical factor of infertility. It involves collection of cervical mucus (by aspiration with a syringe) shortly before the expected time of ovulation and 2 - 4 hours after intercourse. 

Post Coital Test is important and can gives us idea that 1) whether enough semen was delivered to the cervix; 2) sperm are healthy or not 3): the sperm are able to swim through the female reproductive tract and are energetic or not.  Ineffective coital technique, failed ejaculation, poor semen quality, and use of coital lubricants can be toxic to sperm-can produce absence of motile sperm. It is possible that the female produces antibodies to the sperm and these antibodies to destroy sperm. This produces negative or abnormal PCT.

PCT is judged as parameter for success in treatment of infertility. Good PCT means success is imminent. PCT is directly related to success rate in infertility management.

Male Infertility Factor
Amongst primary infertile couple male factor appears to be responsible in 30 to 40 percent. Infertility can result from a condition that is present at birth (congenital) or developed later (acquired).

Common causes for male infertility are: Disease Of Testes, Infection, Testicular Trauma, Defect Or Obstruction In Reproductive tract, Hormone Dysfunction, Previous Medications or infections, Metabolic Disorders, Systemic disease of body, Vericocele. These causes Produces 1) impaired sperm production, 2) impaired sperm delivery, and 3) testosterone deficiency

Allopathic  treatment for Male Infertility Factor is long, time consuming, very expensive and again not to the satisfaction - does not give desired result, also side effects of drugs is much problematic. Combined approach of all alternative medicine can only give desired results in short time and bring satisfactions in sexual relations. Combined approach of all alternative medicine is easy to follow, very safe, no side effects, corrects problems from root cause, gives early results and very economical.

Considerations in Management of Infertility:-
It is helpful when beginning fertility treatment to develop a perfect plan. Patients should be treated as per their own constitution and requirements which varies from individual to individual.

There are many systems of medicine available besides modern conventional allopathic treatment. They are designated as Alternative medicine. It covers a broad range of healing philosophies, approaches, and therapies. Examples of very common alternative medicine are homeopathy, ayurveda, naturopathy, rackii, biochemic, hypnosis…etc. Each system has their own characteristics and benefits to give healing and cure in infertility. Each system of alternate medicine claim’s high for cure on its own way, so it is easy to understand that combine approach can give best result as per one’s expectations.

For any reasons of infertility advantages of various systems of medicine should be combined to offer best approach for the management of childless couple. This may give result faster, with less suffering and in economical way. Combined approach of all alternative medicine can only give desired results in short time. Combined approach of all alternative medicine is easy to follow, very safe, no side effects, corrects problems from root cause, gives early results and very economical.

In my treatment approach patients has not necessary to come personally. It is telemedicine. Alternative medicine is concern with alteration in body function and alternation of body function is reflected in symptoms.
