In vitro fertilization (IVF) or test-tube baby is a process in which eggs are taken out from woman's ovaries, fertilized with sperms and allowed to develop for a few days in a fluid medium in the laboratory; the resulting embryos are then transferred back in to the uterus to establish a pregnancy. The fertilization procedure depends on semen parameters like sperm count, motility, morphology etc. In conventional IVF, eggs are placed with motile sperm in a dish or tube to allow one of the sperms to fertilize the egg on its own. If semen parameters are severely effected, then one sperm is injected directly into the egg under magnification: this procedure is called Intra Cytoplasmic Sperm Injection (ICSI).

Who can benefit from IVF?
IVF is usually considered by couples who have:
Male factor infertility (sperm count or sperm motility is low): only 50,000 to 100,000 sperms are placed in a dish to fertilize an egg in IVF. Hence, IVF is offered to men with mild to moderate problems, when other treatment options have failed.
Female factor infertility

Absent or blocked fallopean tubes: Infections, inflammations and other conditions may cause irreparable damage to the Fallopian tubes. IVF allows for successful fertilization bypassing the problem area.

Endometriosis, when surgery or other therapies have failed.

Pelvic adhession (due to infection, surgery) interfere in the transport of egg by the fallopian tube. These problems are bypassed with IVF.

Infrequent ovulation, when treatment with ovulation-induction has not worked.

Ovarian failure, after menopause: iVF is carried out using donor eggs.

Unexplained infertility: When other simpler treatment options like IUI have failed.

Advanced age: The success rate of infertility treatment falls with advancing age, very rapidly after 38 years. In these women, IVF offers best option compared to wasting precious time on less effective treatment.
Intracytoplasmic Sperm Injection (ICSI)
This is a modification of the IVF technique and is used in situations where only a very small number of sperm are available or when no sperms are present in semen but can be obtained directly from the tested through biopsy or when fertilization failure had occurred in a previous IVF cycle. With this method, an individual sperm is selected and injected directly into an egg using a microscope and micro manipulator.
Who can benefit from ICSI?
ICSI has become a powerful means of overcoming male factor infertility as ICSI needs only one sperm to fertilize an oocyte or an egg.

Severe Oligospermia : very low sperm concentrations

Severe Asthenospermia : very low sperm motility

Teratospermia: less than 4% normal sperms

Obstructive azoospermia : when no sperms are present in semen (Due to congenital absence of vas deferens, failed vasectomy reversal etc.) but are being produced in the testes.

Previous Poor Fertilization with IVF: ICSI can be successfully used in patients with fertilization failure after conventional IVF.
In vitro fertilization Procedure:
The IVF procedure consists of several steps that place over a period of a couple of weeks:

Counseling: Education of the couple about the steps involved in IVF, its risks and benefits, helping them cope up with the stress.

Ovarian stimulation: involves the use of fertility medications to increase the number of eggs (follicles) that develop in the ovaries. The response is monitored with trans vaginal sonography. When the right size is attained, an injection of HCG is given to trigger ovulation. HCG is usually injected at a particular time in the evening.

Egg retrieval: This is carried out approximately 34 to 36 hours after injecting hCG. Eggs are retrieved from the ovary using a very fine needle and sonography guidance user short general anesthesia.

Fertilization: After retrieval, the eggs are combined with sperm in the laboratory with conventional IVF or ICSI.

Embryo transfer: Approximately two days after the retrieval, one to three embryos are placed in the woman's uterus using a thin, flexible catheter inserted through cervix.

Embryos Freezing: Embryos that are not transferred back and are of good quality are stored by freezing them (called cyropreservation).
These can be used later on if the attempt fails or for another pregnancy.
Luteal Support: some form of medication is given to support the growth of the embryo after it has been transferred.
Pregnancy testing: A blood test is carried out approximately tow weeks after the embryo transfer.
