Artificial insemination also called intrauterine insemination (IUI) is defined as the introduction of a non-sperm in the reproductive nature of women in order to achieve pregnancy.

It is ideal for those cases where there is a difficulty or impediment to the passage of sperm through the cervix, either by changes in semen quality or cervical mucus as well as incompatibility between the male gametes and the same mucus.

For Artificial Insemination the patient first undergoes hormonal stimulation to recruit more than one egg, thereby increasing the likelihood of pregnancy. The stimulation lasts on average of between 10 and 12 days.

During this period to assess the growth and development of follicles, they are monitored by ultrasound. When most of the follicles have reached an average size of 18 to 20 mm, a hormone called HCG is injected. This is the hormone responsible for finishing the follicular maturation. About 36 hours post-HCG insemination program, the sperm is deposited into the uterus of the woman.

Insemination is an outpatient procedure that is performed in the operating room without analgesia or anesthesia. This procedure takes about 15 minutes, for this a thin and very soft inert plastic tube called a catéter will be used. This is inserted through the cervix and once inside of the cavity, sperm is deposited into the uterus. The Sperm injection is performed slowly to avoid bloating and uterine contractions occurring that may cause the vagina to eject it.

In vitro fertilization is a method of assisted reproduction addressed by infertile couples. Its purpose is to fertilize eggs and sperm outside the woman's body, when they are unable to do so in its natural place.

The fertilized eggs are kept in incubators for optimal development, this is called embryo culture. The embryo culture requires extensive care and can last from 3-5 days depending on the quality of the embryos.

Thus stages of IVF / ET are:
  1. Ovarian Stimulation. This is accomplished with daily subcutaneous injections of hormones to stimulate the ovaries of the women. The drugs used are follicle stimulating hormone (FSH) and luteinizing hormone (LH). Ovarian stimulation lasts on average between 10 and 12 days. During this period the growth and development of follicles will be assessed using an ultrasound.
  2. Follicular aspiration. This procedure is intended to remove the eggs from inside the follicles. It is performed by an ovarian puncture with a needle inserted through the vagina and guided into the follicle using ultrasound visualization. This is an outpatient procedure that requires local anesthesia and sometimes depending on the patient and the location of the ovaries, the time required in the operating room to perform the procedure is 20-30 min.
  3. Fertilization. Is a process that begins with the entry of sperm to the egg. For fertilization to occur, about 25 000 to 50 000 previously trained sperm of the husband are incubated in the same medium capsule with each egg in the laboratory. The evidence of there being fertilization is given by microscopic visualization of the pronuclei (male and female), 16 to 20 hours after incubation of both gametes.
  4. Embryo transfer. It is an outpatient procedure that is performed in the operating room without analgesia or anesthesia. This procedure takes about 15 minutes and consists of depositing the embryos within the uterine cavity. It uses a thin plastic tube called inert and a very soft catheter. This is inserted through the cervix and once inside of the cavity, the embryos are placed in the uterine cavity. The number of embryos transferred depend upon the quality and age of the patient. So, if the embryos are transferred on the third day, they must continue to develop in the uterine fluid before making contact with the endometrium and begin implantation. In case there is an excess of embryos and these are of good quality they will proceed to their cryopreservation for use in a future cycle.

Intracytoplasmic Sperm Injection (ICSI) is a form of IVF used for infertile couples with multiple factors meaning sperm being unable to penetrate into the egg.

The factors which indicate the ICSI are:
  • Male Factor (oligozoospermia, asthenozoospermia, and azoospermia teratozoospermia).
  • Fertilization failure by conventional IVF.
  • In Vitro Maturation.
  • Use of eggs from Bank.
  • Low number of eggs retrieved in egg retrieval.
  • Presence of anti-sperm antibodies.
  • Under the influence of any of these factors, it is necessary to facilitate fertilization by injecting a single sperm into the egg. This is done using microscopy equipment, equipped with systems that allow its introduction micromanipulation.
The stages of ICSI are similar to IVF, the difference is the way the eggs are fertilized. Thus sperm injection are trained by the embryologist and according to selection criteria, determine which techniques are used to inject sperm into the egg.

The ICSI requires only one sperm for each egg and therefore, the instructions have been extended to include almost all men with severe infertility, including many who in the past would have been regarded as hopeless cases. If sperm are viable, giving their shortcomings can be overcome, over 75% of eggs can be fertilized normally regardless of the quality of sperm. Obstructive azoospermia can also be treated by collecting the sperm directly from the testicles and even immature sperm can be used to produce embryos.

Assisted hatching is a technique that consists of making a small hole in the zona pellucida (outer membrane of the embryo) in order to facilitate the exit of the inner cell mass (ICM), when the embryo is located in blastocyst stage , to implant in the womb.

Naturally, the zona pellucida of the embryo gradually thins and on day 6 of development it is clear to cling to the endometrium of the uterus and thus be implanted. There are eggs that have a thickened zona pellucida, this makes the output of embryo implantation.

These embryos therefore are less likely to follow their development and be implanted.

The indications for this are:
  • Eggs with thickened zona pellucida.
  • Thawed embryo transfer.
  • Women over 37 who have problems getting pregnant.
  • Elevated basal serum FSH.
  • Failure of several cycles of IVF.
  • Slowly developing embryos.
  • Embryos with abnormal color zona pellucida.
