DOCUMENTS REQUIRED FOR SURROGACY
1 Birth certificate Apostille from La Haya (from both intended parents)
2 Non criminal records certificate Apostille from La Haya (from both intended parents)
3 Marriage certificate Apostille from La Haya
4 Prove of residence (register o public record, lease agreement)
5 Photocopy of ID card
6 Attested or certified Passport photocopy
7 Medical tests:
a)Serology: VRDL, HIV, Hepatitis, Chlamydia
b) Spermiogram
c) Karyotype (blood test)
8 Letter signed and adressed to the judge explaining the reasons why you want to be parents (one letter for each intended parent)
9 Job record of social security (both intended parents)

a) Serology: VRDL, HIV, Hepatitis, Chlamydia
b) Spermiogram
c) Karyotype (blood test)
