2,732 babies were born in 2013 in the new birth center at Fondazione Poliambulanza, opened in early 2012. Care during childbirth is focused on promoting the process of labour and delivery as a natural event, allowing women to be free to act spontaneously during their birth and to perform alternative postures, including the possibility of water birth.

The labour and delivery unit allows the woman's partner or significant other to attend the birthing event, and the possibility is offered to use the epidural analgesia service 24/7.

In May 2012 the new Neonatal Intensive Care Unit started its activities. The Unit is comprised of 6 NICU beds for the care or preterm and term neonates with respiratory, neurologic, cardiac, infectious conditions. It is equipped with all main diagnostic techniques (X-ray, cranial, cardiac and abdominal ultrasound, magnetic resonance, computerized tomography, EEG, screening for ROP, and hearing tests) and patient care techniques (conventional ventilatory support and high-frequency ventilation, nitrous oxide, hypothermia, surfactant, total parenteral nutrition, peripheral and central venous catheterization) required by modern and advanced neonatal care with highly professional medical and nursing staff.

Specialized ambulatory care includes medical examinations and all other diagnostic services carried out for outpatients. Approximately 378,000 outpatients used the ambulatory care services of Fondazione Poliambulanza in 2013.

Approximately 68,700 patients visited the Fondazione Poliambulanza Emergency Room with a 5.4%increase from 2012. The Emergency Room facilities, expanded in May 2012 and equipped with 20Short-Stay Observation beds including 6 monitored beds, a CT scanner and a dedicatedX-ray machine, with a new entranceway providing easier access for vehicles, and a newtriage station, stood the test of increased volumes. Together with the development of the facilities, the Emergency Room organization also grew over time, and now counts on 17 physicians, 50 nurses, 13 licensed practical nurses, and 21 nurse’s aides.

DEPARTMENT OF CARDIOVASCULAR MEDICINE

  • Cardiology - Claudio Cuccia, MD
  • Cardiac surgery - Giovanni Troise, MD
  • Vascular surgery - Antonio Sarcina, MD

Patients with cardiovascular conditions can have access to a full range of diagnosis and treatment services in the patient care units comprising the Department of Cardiovascular Medicine at Fondazione Poliambulanza, implementing a highly specialized and multidisciplinary approach. The Department of Cardiovascular Medicine has a dedicated Surgical Suite with 3 operating rooms and 2 Cardiac Catheterization and EP rooms, as well as a dedicated Anaesthesia and Intensive Care Unit with 5 beds.

Approximately 500 patients had a Cardiac Surgery procedure. Areas of excellence characterizing Cardiac Surgery are coronary artery bypass grafting, in 90% of cases without extracorporeal circulation (“beating heart” surgery), with the patient's benefit of less invasiveness and the possibility of spending the immediate post-op period on the regular floor rather than in the Intensive Care Unit (LEss invAsive Surgical Track or LE.A.S.T. pathway), mitral and tricuspid valve repair surgery also performed using the minimally invasive technique, conservative aortic valve surgery (reconstructive and valve-sparing techniques), with the patient's benefit of avoiding anticoagulation therapy and all the problems associated with it. Poliambulanza's Cardiac Surgery Unit is a referral center in Italy and abroad for aortic valve replacement with sutureless bioprosthesis.

In 2013, the Cath Lab and Cardiac Surgery Unit collaborated in performing 20 percutaneous aortic valve replacement (PAVR) or transcatheter aortic valve implantation (TAVI) procedures, an innovative procedure with limited indications for eligible patients with high surgical risk only.

The Italian Agency for Regional Healthcare Services (AGENAS) published the indicators concerning the quality of the care and services delivered by Italian hospitals: Poliambulanza's data about mortality in the first month after heart valve repair or replacement surgery is the best result nationwide.

Approximately 840 patients had a Vascular Surgery procedure in 2013. Both traditional surgery techniques and endovascular approaches (80 cases of aortic and thoracic endoprostheses in 2013) were used for arterial diseases (2/3 of total patients). Venous disease (managed in 95% of cases on an outpatient basis) is treated using both traditional stripping techniques and laser-induced thermal ablation techniques.

30-day mortality is considered a good measure for the entire patient care process that involves – besides vascular surgeons – cardiologists, radiologists, anaesthesiologists and intensivists, and Poliambulanza's data (69 procedures in 2012) is one of the best in Italy.

DEPARTMENT OF SURGERY / DEPARTMENT OF ONCOLOGY/ WOMEN HEALTH DEPARTMENT

  • General Surgery - Edoardo Rosso, MD
  • Urology - Michelangelo Tosana, MD
  • Gynecology - Renato Brighenti, MD & Federico Quaglia, MD
  • Oncology - Alberto Zaniboni, MD
  • Digestive Endoscopy Unit - Alessandro Paterlini, MD
  • Pathology - Fausto Zorzi, MD
  • Radiation Therapy - Mario Bignardi, MD

At Fondazione Poliambulanza, patients with cancer conditions can find a team of highly experienced professionals, interdisciplinary competences, leading-edge technologies, and clinical research throughout a 360° integrated care pathway that includes diagnosis, cancer surgery, reconstructive surgery, chemotherapy, radiation therapy, psychological support as well as all the most appropriate medical care and treatment.

Approximately 1,530 cancer surgery procedures were performed in 2013, 361 of which relating to conditions of the digestive system (chiefly affecting the colon, rectum, stomach, and liver), 276 of the bladder, 324 of the breasts, 141 of the female reproductive system, 47 of the brain, and 7 of the oral cavity. This activity comprises the main workload of the General Surgery, Urology, Gynaecology, Neurosurgery, and ENT Units.

Breast cancer is the most frequent neoplasm in women, and can be more easily curable when it is detected early. For this reason, Poliambulanza formed the Breast Unit, a multidisciplinary integrated care unit for the diagnosis, treatment, and rehabilitation of breast diseases provided by a dedicated team of radiologists, surgeons, pathologists, plastic surgeons, radiation therapists, oncologists, psycho-oncologists, and physical therapists. The aim is to ensure timely diagnosis for all patients requiring urgent testing to further study their morphological findings by means of cytological/histological examinations. The diagnostic phase, coordinated by the multidisciplinary care team, ensures thorough and safe diagnosis, and immediate planning for the treatment strategy.

In 2013 the Medical Oncology Unit of Poliambulanza followed 1,880 patients. The participation of the Oncology Unit in clinical trials is particularly significant, in collaboration with the leading pharmaceutical companies and with major international organizations. Thus, patients who are enrolled on the various research studies according to rigorous protocols validated by the LHA Institutional Review Board, can be offered the most advanced drugs that are being used by the best cancer centers worldwide.

Beginning on February 1st, 2011, Poliambulanza patients can also use the services provided by the new Radiation Oncology Center, named after Cav. Guido Berlucchi.

In 2013, the Radiation Therapy Unit performed 704 treatment cycles taking care of a total of 627 patients. A considerable portion of these patients (36%) were treated with highly-advanced intensity-modulated or stereotactic techniques (IMRT, VMAT, SBRT, SRS/SRT) allowing cancer to be cured with the utmost precision available nowadays. Approximately 73% of patients came from the internal Patient Care Units of Fondazione, while the remaining 27% were referred by other community organizations (136 patients compared with 66 in 2012) or were self-referred patients.

In 2013, 13 patients were treated with Stereotactic Radiation Therapy of the Brain (also called “Radiosurgery”), a very complex and niche technique that many Radiation therapy units do not perform. In the first few months of 2013, the 100th stereotactic treatment was reached (19 brain treatments and 81 body treatments, including 31 liver and 27 lung treatments).

Poliambulanza Foundation Hospital in Brescia, Italy - line8

DEPARTMENT OF ORTHOPEDICS AND TRAUMATOLOGY / DEPARTMENT OF REHABILITATION MEDICINE

  • Orthopedics and Traumatology - Flavio Terragnoli, MD
  • Specialized Rehabilitation - Gianna Santus, MD

A total of 4,380 patients were admitted to the Orthopaedics and Traumatology Unit in 2013 (3,319 on an inpatient basis and 1,061 for ambulatory surgery) for joint replacement surgery (829 patients), arthroscopy (shoulder, elbow, wrist, hip, knee, ankle), foot surgery, hand surgery, and for the treatment of genetic or post-traumatic bone defects with external fixators. Immediately after surgery, post-op patients are enrolled on a rehabilitation program managed by the physical therapy team supporting the ward.

186 patients had urgent hip replacement surgery as a result of a femoral neck fracture (35% of total hip replacement procedures), a particularly frequent traumatic event in elderly age, mostly caused by chronic bone conditions (e.g. senile osteoporosis). The surgical strategy depends on the type of fracture and on the patient's age, and indicated surgical procedures are fracture reduction and hip replacement. All international guidelines show that long wait times for surgery put the patient at increased risk of mortality and disability – consequently, general recommendations provide for patients with femoral neck fracture to be operated on as soon as possible, possibly within 24 hours of admission to hospital, because several complications may frequently affect the immobilized elderly. Poliambulanza data (93.4% of patients operated on within 48 hours) is the best result nationwide.

DEPARTMENT OF HEAD AND NECK SURGERY

  • Neurosurgery - Massimo Gandolfini, MD & Oscar Vivaldi, MD
  • Otolaryngology - Ugo Moz, MD
  • Ophtalmology - Vincenzo Miglio, MD
  • Neurology - Edoardo Donati, MD

The Neurosurgery unit admitted patients with malignant and benign brain tumours (112), cerebrovascular diseases (brain haemorrhage and ischemia, 85 surgical procedures), cervical and lumbar disc herniation (280 procedures in 2013), degenerative and/or post-traumatic spinal instability (169 spine stabilization procedures).

The Parkinson's Disease and Movement Disorders Center has been active within the Department of Neurology and Neurosurgery since 2001. The Department can now boast much long-standing experience of the treatment of the most advanced forms of complicated Parkinson's disease, when standard drug therapy is no longer capable of adequately controlling symptoms and of ensuring satisfactory quality of life for the patient. Deep brain stimulation has been performed since January 2005 to implant neurostimulators in the subthalamus (10 procedures in 2013). The successful outcome of this procedure requires integrated stereotactic and functional neurosurgery and neurophysiology competencies.

Beginning in September 2011, a new organization and the purchase of a state-of-the-art biplane angiography system made it possible to widen the range of services provided by Poliambulanza in the field of interventional neuroradiology, including endovascular treatment of vascular malformations and brain aneurysms, among others.

Poliambulanza Foundation Hospital in Brescia, Italy - line7

DEPARTMENT OF RADIOLOGY AND DIAGNOSTIC IMAGING

  • Radiology Unit - Silvia Magnaldi, MD
  • Nuclear Medicine - Savelli Giordano, MD

2013 was the second full year of activity of the new Nuclear Medicine Unit, that can perform PET-CT (positron emission tomography integrated with spiral computed tomography), SPECT (gamma camera), and SPECT-CT (gamma camera integrated with computed tomography) scans, thus carrying out all diagnostic imaging studies using radioactive isotopes.

In 2013, besides PET scanning using FDG additional PET imaging techniques were implemented, namely PET studies using F-labelled Choline (especially useful for imaging of prostate cancer), F–DOPA (for medullary thyroid cancer and pheochromocytoma), and Florbetapir F-18 (for diagnosing neurodegenerative conditions like Alzheimer's disease), thus widening the range of available PET imaging studies.

Approximately 4,700 scans were performed for both outpatients and inpatients in 2013, including 1,975 PET scans using imaging equipment that allows patient doses to be significantly reduced by approximately 40% on average as compared with recommended dose limits or DRLs, thus reducing radiation "risks" for patients.
