Treatments and Procedures

PROSTATE CANCER: - ROBOTIC RADICAL PROSTATECTOMY

Ideal treatment of Prostate Cancer is ROBOTIC RADICAL PROSTATECTOMY, which is Surgical Removal of the entire prostate gland and curing the Prostate Cancer using the Da Vinci Platform and Robotic System.

Robotic Radical Prostatectomy is the gold standard to treat the early stage; organ confined prostate cancer before it has spread to other parts of the body.

Advantages of robotic techniques:

Because the display system of the da Vinci projects the image in the direction of the surgeon's hands, the optically correct hand-eye coordination is restored. This is more difficult with laparoscopy, in which the camera is sometimes offset to the plane of dissection.

The 11-mm telescope in the da Vinci system is a combination of two 5-mm optical channels (one for the right eye and one for the left eye), which have 2 separate 3-chip–charged coupling devices in the camera head. The 2 images are displayed to provide 3-dimensional (3-D) stereoscopic vision to the surgeon, providing depth perception lacking in laparoscopy. The conventional laparoscopic technique does not provide a 3-D depth of view.

The movements of the robotic system are intuitive (i.e., a movement of the master control to the right causes the instrument to move to the right), as opposed to the counterintuitive movements in laparoscopy with fulcrum movement effects (i.e., movement of the laparoscopic instrument to the right by the surgeon causes the tip of the instrument to move to the left inside the patient's body).

The robotic systems provide increased precision by filtering hand tremors, providing magnification (10X or 15X), and providing scaling for the surgeon's movements (a 1:3 scaling means that a 3-in movement of the master is translated into a 1-in movement of the instrument tip).

The robotic instruments have articulated tips, which permit 7° of freedom in movements (i.e., they mimic human wrist movements, including rotation), which is unlike laparoscopy, with which only 4° of freedom are permitted.

Green Light Laser TURP

This laser, also known as a potassium-titanyl phosphate (KTP) laser, yields a green visible light beam of 532 nm. It is a High power Laser used to vaporize the obstructing part of the prostate, thus improving the urine flow.

Advantages of Green Light PVP over the conventional TURP:

• Lesser bleeding
• Can be safely used in cardiac patients and
• Can be safely used in patients taking chronic oral anticoagulants and anti-platelet therapy
• Lesser transurethral resection (TUR) syndrome, so very safe for cardiac and elderly patients
• Less perioperative pain

Kidney Stone- Holmium Laser Treatment with Flexible Ureteroscopy

Dr Ashish Sabharwal is an expert in Laser Treatment of Kidney stones (Flexible Ureteroscopy - Holmium Laser Lithotripsy). In the Holmium Laser Lithotripsy, Light energy of 2150 nm is delivered in a pulsatile fashion through low–water density quartz fibers.

Advantages of Laser treatment of Kidney stones:

• Minimal risk of surrounding thermal injury exists
• Minimal fragment migration and retrograde propulsion
• Its ability to fragment all stones regardless of composition or size, and its ability to deliver higher energy settings even through the smallest of delivery fibers.
• Hard stones in difficult locations (eg, lower pole caliceal calculi)
• With the help of flexible ureteroscopy , even stones inside the kidney can be removed without making a hole in the kidney (PCNL).

Erectile Dysfunction - Penile Prosthesis Implantation

Penile Prosthesis Implantation is the ideal treatment of Erectile Dysfunction. Diabetic and cardiac patients after age of 50 years often suffer from erectile dysfunction. In the early stages, erectile dysfunction is treated by medications and vacuum therapy system. But in the last stages of erectile dysfunction, non operative treatments become ineffective. A penile implant remains a reliable form of therapy in highly motivated men when other treatments are unsuccessful. Almost all men with penile implants express satisfaction.

Two types of penile implants are available: a semirigid or malleable rod implant and an inflatable implant.

• Semirigid or Malleable Penile Implants

With the semirigid or malleable rod implant, 2 matching cylinders are surgically implanted into the penis. To achieve an erection, you manually adjust the position of the penis. This implant provides enough rigidity for penetration and rarely breaks.

Advantages of the semirigid or malleable rod implant include:

• Relatively few complications

• No moving parts

• Least expensive implant

• 70-80% success rate

• Highly effective

• 3 pc Inflatable Implants

The inflatable implant consists of:

Two cylinders surgically inserted into the penis. A pump, placed in the scrotum.

A reservoir, contained either within the cylinders or in a separate reservoir placed beneath the tissue of the lower abdomen.

With an inflatable implant, you squeeze the pump placed within the scrotum to inflate the cylinders. The pump moves fluid from the reservoir implanted in the lower abdomen to the cylinders implanted in the penis. The expansion of these cylinders creates an erection.
