GERD
Gastro Esophageal Reflux Disease (GERD) is quite a common gastrointestinal problem in our country. GERD results due to movement of gastric contents and acids into the esophagus. Normally food passes from mouth to stomach through esopagus (food pipe) and food does not go back into the food pipe due to pressure of lower esophagus Sphincter (LES). Excessive, Frequent relaxation of LES causes GERD. Hiatus Hernia associated with this condition allows part of stomach to protrude into chest cavity. This condition contributes to failure of sphincter of lower esophagus.
Gall Bladder stones
Stones which are formed insied the gallbladder lumen are known as Gallstones. These stones vary in number size and color. They are made of bile salts, bile pigments and cholestrol.
Appendicitis
Appendicitis is an inflammation of appendix (a painful swelling and infection). If it is not treated, it can eventually burst and infection can spread throughout the abdomen.
Hernia
A hernia occurs when there is a bulge or protrusion of the abdominal viscera through the weak muscles of the abdominal wall and there is a small balloon like swelling. This bulge is seen more prominently with weight lifting and any kind of straining. Hernia may be seen at birth or can develop in adult life. Most of the times, it becomes bigger with time and age, and usually requires surgical treatment.

What is GERD?

Symptoms of GERD
The most common symptoms are:

Heart Burn (Experienced as a retrosternal burning discomfort).

Indigestion.

Regurgitation of food in mouth on lying down specially during sleep.

Dysphagia and difficulty in swallowing.
-
Difficult sleep after eating.
-
Atypical symptoms can be chest pain, wheezing, hoarseness, chronic cough, chocking etc.
If untreated GERD can cause ulcer formation, bleeding, narrowing of the esophagus that causes difficulty in swallowing. In some patients this may lead to "barrett's esophagus" and cancer if untreated. Also there can be aspiration pneumonia, asthma, pulmonary fibrosis.
For the above mentioned, surgery is a curative treatment. Surgery can reconstruct the LES as well as repair the Hiatus Hernia.

Antireflux surgery is done by making three ( 3-5-10 mm sized) tiny holes, which are made in the upper abdomen, through these holes, instruments and the laparoscope (telescope) are passed, the telescope is connected to a special camera which allows a direct view of the stomach and esophagus, as well as other abdominal organs. This involves wrapping a portion of stomach (fundus) around LES with reduction of a Hiatial hernia if present. This constructs a valve like mechanism to re-establish gastroesphageal junction competence. This creates a barrier to the reflux of gastric contents, so it provides relief of the symptoms and prevents the complications associated with GERD.

How Gallstones are formed?
It is still not clear why some people develop gallstones, however risk factors include
Symptoms

Galldtones when formed can give rise to severe pain in right upper abdomen, vomiting, indigestion, occasionally fever and may be jaundice.

Gall stones pain often occurs after eating fried meals.

Pain due to gall stones can mimic like heart attack, so accurate diagnosis is very important.
-
Diagnosis can be made by simple ultrasound scan done in a fasting state.
-
Gallstones can cause jaundice and pancreatitis if they get trapped in the bile ducts.
Treatment
There is no treatment for gall bladder stones. There may be short term success with dietary adjustments and drugs but eventually symptoms will continue.
Surgical removal of Gall bladder (Laparoscopic Cholecystectomy) is the time honoured, safest and gold standard treatment all over the world.
This surgery can be done in every patient inclusing patients with previous abdominal operation, cardiac, asthmatic and diabetic patients also.
This surgery can be done as a day care procedure in young and selected patient.
Procedure
Under General anesthesia, the surgeon makes few tiny holes in the abdomen and inserts a telescope with a mounted video camera into the abdomen. As a result a magnified image of patient's internal organs are displayed onto a video monitor, Through other holes surgical instruments are inserted and by watching the moniotr surgeon carefully separates the gallbladder from the liver ducts and vessels. Gall bladder is the removed through one of the small incisions. All the incisions are closed with one or two stiches.
In a small number of patients, if laparoscopic surgery is not possible then gall bladder is removed by open surgery. In open surgery, the surgeon have to make 5-7" incision in the abdomen to remove the gall bladder. Open surgery is required in less than 0.2% gall bladder operation at our hospital.

Appendicitis
What is Appendicitis?
Appendicitis is an inflammation of appendix (a painful swelling and infection). If it is not treated, it can eventually burst and infection can spread throughout the abdomen.
Symptoms
There are many typical symptoms of Appendicitis; most common is severe abdominal pain, i.e. pain around belly button which occurs suddenly and gets worse when moving around, taking deep breadths, sneezing or coughing. Further symptoms include nausea, vomiting, loss of appetite, abdominal swelling and painful urination.
Treatment
Open Technique:
Through a single incision in the lower right area of the abdomen, appendix is removed.
Laparoscopic Appendicectomy:
Under general anesthesia, three (3-5-10 mm sized) tiny holes are made in the lower abdomen, through these holes, instruments and the laparoscope (telescope) are passed; the telescope is connected to a special camera which allows a direct view of the appendix as well as other abdominal and pelvic organs. With other instruments, by watching the inside picture of the organs on the screen, appendix is separated, ligated at its base and then removed through one of the holes.

Complications of acute Appendicitis
If appendix is not removed during appendicitis then it can lead to generalized abdominal infection, appendicular abcess formation, rupture of appendix and also gangrene.
Hernia

Symptoms of Hernia
The hernia appeares as bulge under the skin which becomes more prominent on standing and straining and disappeares on lying down. THe swelling can be pushed back with gurgling sound on lying down. At times gurgling sound of the intestine can be heared or felt. Mild discomfort or pain can occur but hernias are usually painless. Severe continuous pain, redness and tenderness are signs that hernia are getting obstructed and strangulated. These symptoms are of serious concern, and one should contact the surgeon immediately.
What are the causes of hernia.
Risks of Hernia
The hernias can get obstructed or strangulated which causes abdominal pain, nausea, vomiting and unable to pass wind. These symptoms require urgent attention and immediate surgery. Otherwise Hernia should be treated as early possible. The surgical results with smaller Hernias are much better as compared to large Hernias.
How to treat Hernia:
Open Surgery
In this method, a skin incision of desireable length is given and the Hernia Sac is dissected after reaching the defect. The sac is pushed back and the remaining defect in the muscle layers is sutured together. Now a days in open method after the content of the hernia sac have been pushed back in to the abdominal cavity, the site of the hernia is covered and with a mesh and wound is closed. Using a mesh allows tensiion free treatment of Hernias.
Laparoscopic/ Endoscopic Hernia Repair
Three (5-10 mmm sized) incisions are made depending on the size of the hernia. Through these incisions instruments and laparoscope (telescope) are passed; the telescope is connected to a special camera which allows the surgeon to see the hernia and surrounding area on a video monitor. With other instruments, by watching the inside picture of the organs on the screen, surgeon is able to fix a surgical mesh over the hernia defect and held in place with small surgical staples.
