[00:00:00] Hello everyone. Many of my patients wonder, after surgery, how they will use their penile implant. That's why I decided to prepare this video.
[00:00:10] We need these videos ready so we can send them to our patients.
[00:00:22] Especially after surgery, if your implant was placed through an infrapubic incision (an incision above the penis), you should begin inflation and deflation exercises after approximately 45 days.
[00:00:36] If your incision is underneath, you can usually begin these cycling exercises comfortably from the 10th day onward.
[00:00:48] In this video, I will briefly explain everything. First, it is very important that you become familiar with the components of the implant, how it works, where to press, and the key points you need to know.
[00:01:06] What you see here is a three-piece inflatable penile implant, consisting of a reservoir, two cylinders inside the penis, and a pump.
[00:01:17] The pump itself has two sections: the inflation pump at the bottom, and the deflation button at the top. It is important to recognize these parts clearly.
[00:01:34] I'm showing them up close because once they are beneath your skin, you'll need to identify the pump and the deflation button above it.
[00:01:48] Inside your penis, there are two inflatable cylinders. As you can see, the pump is usually placed discreetly between the testicles where it is not easily noticeable.
[00:02:01] You generally won't feel the tubing much, because it is positioned behind the scrotal tissues. The fluid reservoir may be placed on either the right or left side. If there is concern about a hernia, the reservoir is placed on the opposite side.
[00:02:25] Now, how does it work? I'll first demonstrate using this model, and then show you a realistic simulation. In the beginning, you may even need both hands to hold the pump.
[00:02:58] At first, using only two fingers may not be easy. So use three or four fingers to get a secure grip. As I begin pumping, you'll notice the reservoir gradually empties while the cylinders begin filling with fluid. This is an LGX700 model, designed to expand in both length and girth.
[00:03:24] It has a main cylinder length of approximately 21 cm. To fully inflate it, you generally need around 10 to 15 squeezes. You do not use all of the fluid in the reservoir.
[00:03:48] Since this is outside the body's tissue constraints, the demonstration model can continue inflating indefinitely. This helps demonstrate how the implant increases both length and girth. Compared to the yellow marker, you can see an increase in length of approximately 1.5 to 2 centimeters. The penis becomes firmer, thicker, and ready for sexual intercourse.
[00:04:15] During your cycling exercises, after inflation, keep the implant inflated for about 10 to 15 minutes.
[00:04:39] Inflate it until it feels as firm as possible and you can no longer comfortably squeeze the pump. It may be more difficult at first, but it becomes easier over time. After waiting 10 to 15 minutes, the cycling session is complete, and it's time to deflate the implant.
[00:05:01] Deflation is simple. Locate the square-shaped deflation button on top of the pump. Place one finger behind the pump and press the raised deflation button for about three seconds. You'll notice the implant beginning to deflate automatically.
[00:05:31] During deflation, you may feel a slight vibration. If you'd like the penis to soften faster, gently squeeze the shaft externally. This speeds up the return of fluid to the reservoir, making the penis soft again.
[00:05:58] One very important point: There is a valve system. After deflation, gently press the pump halfway once to close the valve and reset the mechanism.
[00:06:14] Now let's look at the realistic simulation. This simulator contains the exact same implant shown earlier. Here are the testicles, and between them you can easily feel the pump.
[00:06:34] As you pump, fluid moves from the abdominal reservoir into the cylinders inside the penis. The penis gradually becomes firm, increases in size, and extends forward.
[00:06:54] Immediately after surgery, swelling and postoperative edema may temporarily limit expansion. As healing progresses, inflation becomes easier. Once you can no longer comfortably squeeze the pump, stop pumping. The implant is now firm enough for sexual intercourse.
[00:07:24] Wait another 10 to 15 minutes. Performing this cycling exercise once or twice daily helps the implant settle properly while healing and promotes healthy tissue formation around the implant.
[00:07:42] To deflate, place your finger behind the pump, press the deflation button for three seconds, then release it. The implant will begin deflating automatically. If deflation seems slow, gently squeeze the penis to speed up fluid return.
[00:08:08] That's the basic operation. There are a few important tips, and if you follow them, using the implant becomes much easier. Finally, remember: After fully deflating, give the pump one gentle half-press to close the valve.
