Cell Assisted Lipotransfer for Breast Augmentation in Thailand

What is CAL?
According to Dr. Kotaro Yoshimura, M.D., from the Department of Plastic Surgery, University of Tokyo School of Medicine, and creator of CAL for breast augmentation, CAL is an “autologous adipose-derived stem (stromal) cell (ASC) supplementation that is used in combination with lipoinjection.” When it is used for breast augmentation, autologous fat cells (derived from the patient) are extracted thru liposuction and are then processed by collagenase treatment and centrifugation. The fat extract (that is rich in stem cells) is recombined with half of the remaining fat sample to increase its potency. After the enriching process, the fat cells are introduced thru injection to the breast tissue.
Although there is nil chance of rejection with fat grafts because the fats cells were derived from the patient’s body, there is still risk of interference with future breast cancer diagnosis and risk of tissue hardening (or calcification). To remedy the risks, Dr.Yoshimura thought of enriching the fat cells with high concentrates of stem cells – to regenerate fats cells which in turn preserves and stimulates new growth of blood vessels. Dr.Yoshimura believed that by increasing the fat cell concentration, he could enhance the survival rate of the fatty tissue and lessen post-operative atrophy (deterioration).
Best Candidates for CAL
To date, Dr. Yoshimura has performed this procedure on more than 400 women. Some candidates included women who had breast reconstruction after a mastectomy while others underwent CAL after the removal of their breast implants.
“Women who underwent radiation treatment and mastectomy are often in need of breast reconstruction but using an implant for the procedure may only raise risks for complications,”says Dr. POONPISSAMAI SUWAJO, CAL specialist at Nirunda Clinic. “Capsular contracture (scar tissue that may form and harden) and the side effects of radiation treatment may make me think twice if a client wants to have an implant placed as part of her breast augmentation or reconstruction procedure,” she explains. “This is where CAL comes in. At least now, we can provide another option that’s natural and side-effect free because we are using the patient’s own fat stem cells.”
“Aside from breast augmentation and reconstruction, CAL may also be an option for women who want to have an implant removal/replacement procedure and have CAL instead of a silicone implant, added Dr. CHATPONG SASTARASADHIT, CAL and Plastic Surgery Specialist at Nirunda.

Full CAL vs. Mini CAL at Nirunda
There are two kinds of CAL that is offered at Nirunda: Full CAL and Mini CAL. The CAL Technique is also referred to as natural breast augmentation or California breast lift. In this method, Stem Cell harvesting methods are employed to harvest fat from the patient’s body such as such as the abdomen and thighs. After harvesting the fat cells, it is centrifuged and transformed to a stem cell-rich mixture before it is introduced to the patient’s breast thru injection. After the procedure, the patient is advised to wear a special brassiere to keep the injected fat in its position.
The difference of the Full CAL from the Mini CAL is that the latter is an option for lean or slim patients. The Mini CAL is performed under local anesthesia and involves a shorter processing time. “Overall, breast augmentation thru CAL is a minimally invasive procedure, reiterates Dr. Suwajo. “It involves less pain, its quick (more or less an hour to perform including liposuction) and best of all, it achieves your desired breast size – naturally thru your own fat cells!”
