COCHLEAR IMPLANTATION SURGERY

What is a cochlear implant?

A cochlear implant is a surgically implanted device that helps overcome problems in the inner ear, or cochlea. The cochlea is a snail-shaped, curled tube located in the area of the ear where nerves are contained. Its function is to gather electrical signals from sound vibrations and transmit them to your auditory nerve (or hearing nerve). The hearing nerve then sends these signals to the brain, where they're translated into recognizable sounds.If important parts of the cochlea aren't working properly and the hearing nerve isn't being stimulated, there's no way for the electrical signals to get to the brain. Therefore, hearing doesn't occur. (Sometimes referred to as nerve deafness, this is called sensorineural hearing loss.) By completely bypassing the damaged part of the cochlea, the cochlear implant uses its own electrical signals to stimulate the auditory nerve, allowing the person to hear.

Who uses cochlear implants?

Cochlear implants are designed to help severely to profoundly deaf adults and children who get little or no benefit from hearing aids. Even individuals with severe or profound "nerve deafness" may be able to benefit from cochlear implants.

Lymphatic Bypass Surgery

Ekol Hospitals is one of just a few centers in the country offering lymphovenous bypass and lymph node transfer, innovative surgical techniques designed to reduce swelling and alleviate discomfort for patients with lymphedema.

Lymphedema is a disorder in which lymph fluid accumulates, leading to chronic swelling. It is a common complication following lymph node removal or radiation therapy as part of cancer treatment. The condition can develop from a few days to a few years after treatment and usually affects the arms or legs. While there is no cure, standard care for lymphedema includes physical therapy to decrease pain and improve mobility as well as the use of pressure garments to keep lymph fluid moving.

If the condition worsens and becomes difficult to manage, lymphovenous bypass or vascularized lymph node transfer surgery may help relieve the problem:

  • Lymphovenous bypass - Under a microscope, surgeons use minute instruments to connect blocked lymphatic vessels (as small as 0.3 mm in diameter) to a nearby vein. The bypass allows excess lymph fluid to flow more freely.
  • Lymph node transfer - Healthy vascularized lymph nodes (lymph nodes that have a rich blood supply) are microsurgically transplanted to an area of lymphatic injury to reestablish lymphatic connections.

These surgical procedures are usually more effective for swelling in the arms than in the legs and outcomes are better in the early stages of lymphedema. While results vary, many patients report that their arm or leg feels lighter and more comfortable after the undergoing the treatment.

Throat (Laryngeal) Cancer

Throat cancer refers to cancerous tumors that develop in your throat (pharynx), voice box (larynx) or tonsils. Your throat is a muscular tube that begins behind your nose and ends in your neck. Your voice box sits just below your throat and is also susceptible to throat cancer. The voice box is made of cartilage and contains the vocal cords that vibrate to make sound when you talk. Throat cancer can also affect the piece of cartilage (epiglottis) that acts as a lid for your windpipe. Tonsil cancer, another form of throat cancer, affects the tonsils, which are located on the back of the throat.

Surgery

The types of surgical procedures you may consider to treat your throat cancer depend on the location and stage of your cancer. Options may include:

  • Surgery for early-stage throat cancer. Throat cancer that is confined to the surface of the throat or the vocal cords may be treated surgically using endoscopy. Your doctor may insert a hollow endoscope into your throat or voice box and then pass special surgical tools or a laser through the scope. Using these tools, your doctor can scrape off, cut out or, in the case of the laser, vaporize very superficial cancers.
  • Surgery to remove all or part of the voice box (laryngectomy). For smaller tumors, your doctor may remove the part of your voice box that is affected by cancer, leaving as much of the voice box as possible. Your doctor may be able to preserve your ability to speak and breathe normally. For larger, more-extensive tumors, it may be necessary to remove your entire voice box. Your windpipe is then attached to a hole (stoma) in your throat to allow you to breathe (tracheotomy). If your entire larynx is removed, you have several options for restoring your speech. You can work with a speech pathologist to learn to speak without your voice box.
  • Surgery to remove all or part of the throat (pharyngectomy). Smaller throat cancers may require removing only part of your throat during surgery. Parts that are removed may be reconstructed in order to allow you to swallow food normally. Surgery to remove your entire throat usually includes removal of your voice box as well. Your doctor may be able to reconstruct your throat to allow you to swallow food.
  • Surgery to remove cancerous lymph nodes (neck dissection). If throat cancer has spread deep within your neck, your doctor may recommend surgery to remove some or all of the lymph nodes to see if they contain cancer cells.

Surgery carries a risk of bleeding and infection. Other possible complications, such as difficulty speaking or swallowing, will depend on the specific procedure you undergo.

 

Snoring and Sleep Apnea

Snoring is the hoarse or harsh sound that occurs when your breathing is partially obstructed in some way while you're sleeping. Sometimes snoring may indicate a serious health condition. In addition, snoring can be a nuisance to your partner.

As many as half of adults snore sometimes. Snoring occurs when air flows past relaxed tissues in your throat, causing the tissues to vibrate as you breathe, which creates those irritating sounds. Lifestyle changes, such as losing weight, avoiding alcohol close to bedtime or sleeping on your side, can help stop snoring.

In addition, medical devices and surgery are available that may reduce disruptive snoring. However, these aren't suitable or necessary for everyone who snores.

To treat your condition, your doctor likely will first recommend lifestyle changes, such as losing weight, avoiding alcohol close to bedtime and changing sleeping positions. If lifestyle changes don't eliminate snoring, your doctor may suggest:

  • Oral appliances. Oral appliances are form-fitting dental mouthpieces that help advance the position of your tongue and soft palate to keep your air passage open.

If you choose to use an oral appliance, visit your dental specialist at least once every six months during the first year, and then at least annually after that, to have the fit checked and to make sure that your condition isn't worsening. Excessive salivation, dry mouth, jaw pain and facial discomfort are possible side effects from wearing these devices.

  • Continuous positive airway pressure (CPAP). This approach involves wearing a pressurized mask over your nose while you sleep. The mask is attached to a small pump that forces air through your airway, which keeps it open. CPAP (SEE-pap) eliminates snoring and treats sleep apnea.

Although CPAP is the most reliable method of treating obstructive sleep apnea, and it's effective, some people find it uncomfortable or have trouble adjusting to the noise or feel of the machine. Your doctor may be able to make adjustments to the device if you're having trouble adjusting to the machine, such as adding a heated humidifier or nasal pillows, that might help make you more comfortable.

  • Palatal implants. In this procedure, known as the pillar procedure, doctors inject braided strands of polyester filament into your soft palate, which stiffens it and reduces snoring. Palatal implants don't have any known serious side effects; however, the benefits and safety of the procedure are still being studied.
  • Traditional surgery. In a procedure called uvulopalatopharyngoplasty (UPPP), you're given general anesthetics and your surgeon tightens and trims excess tissues from your throat — a type of face-lift for your throat. The risks of this procedure include bleeding, infection, pain and nasal congestion.
  • Laser surgery. In laser-assisted uvulopalatopharyngoplasty (LAUPPP), an outpatient surgery for snoring, your doctor uses a small hand-held laser beam to shorten the soft palate and remove your uvula. Removing excess tissue enlarges your airway and reduces vibration. You may need more than one session to get your snoring under control.

Laser surgery and palatal implants aren't generally recommended as treatment for sleep apnea, because they haven't been proved effective for sleep apnea. Possible risks from these procedures include pain, infection, bleeding and nasal congestion.

  • Radiofrequency tissue ablation (somnoplasty). In this outpatient procedure, you'll be given local anesthetic. Doctors use a low-intensity radiofrequency signal to shrink tissue in the soft palate to help reduce snoring. The effectiveness of this newer procedure needs further study. Generally, this procedure is less painful than other types of snoring surgery.

Sleep apnea is a potentially serious sleep disorder in which breathing repeatedly stops and starts. You may have sleep apnea if you snore loudly and you feel tired even after a full night's sleep.

There are two main types of sleep apnea:

  • Obstructive sleep apnea, the more common form that occurs when throat muscles relax
  • Central sleep apnea, which occurs when your brain doesn't send proper signals to the muscles that control breathing

If you think you might have sleep apnea, see your doctor. Treatment is necessary to avoid heart problems and other complications.

Your doctor may make an evaluation based on your signs and symptoms or may refer you to a sleep disorder center. There, a sleep specialist can help you decide on your need for further evaluation. Such an evaluation often involves overnight monitoring of your breathing and other body functions during sleep. Home sleep testing is gaining in popularity, because it's often easier for you and less expensive. Tests to detect sleep apnea may include:

  • Nocturnal polysomnography. During this test, you're hooked up to equipment that monitors your heart, lung and brain activity, breathing patterns, arm and leg movements, and blood oxygen levels while you sleep.
  • Home sleep tests. In some cases, your doctor may provide you with simplified tests to be used at home to diagnose sleep apnea. These tests usually involve measuring your heart rate, blood oxygen level, airflow and breathing patterns. If you have sleep apnea, the test results will show drops in your oxygen level during apneas and subsequent rises with awakenings. If the results are abnormal, your doctor may be able to prescribe a therapy without further testing. Portable monitoring devices don't detect all cases of sleep apnea, so your doctor may still recommend polysomnography even if your initial results are normal.

If you have obstructive sleep apnea, your doctor may refer you to an ear, nose and throat doctor (otolaryngologist) to rule out any blockage in your nose or throat. An evaluation by a heart doctor (cardiologist) or a doctor who specializes in the nervous system (neurologist) may be necessary to look for causes of central sleep apnea.

Vocal cords are a pair of structures in our throat (our larynx) that vibrate. They are the source of all the sound we produce. When we are hoarse, something has changed about those vibrations, and the goal of a physician should be to identify the cause of that hoarseness.

Interestingly, there are only two ways to be hoarse:

  1. Air is leaking when you do not want it to leak.
  2. The vocal cords are vibrating irregularly.

In order to diagnose a vocal cord problem that causes hoarseness, we need to look for one of these two findings. Ideally, a physician will look with a camera at the vocal cords and identify either a gap which is leaking air between the vocal cords, or he will find some asymmetry between the cords causing them to vibrate at different pitches. In every patient who complains of hoarseness, we will find one of these two problems. Fortunately, it is very easy to see air leaking or vocal cords vibrating irregularly, at least with the technology available today. Under this section, diagnose, you will find details on how patients are examined and how the cause of hoarseness is identified.

  • Injection laryngoplasty
    • an office procedure to move or increase the bulk in a vocal cord.
  • Medialization laryngoplasty or Augmentation laryngoplasty
    • an external procedure to bulk up or move over one or both vocal cords. It is also known as a thyroplasty or Isshiki Type I procedure as the original description of the procedure was by Dr. Isshiki and he named the procedure after the thyroid cartilage on which he was operating.
  • Microlaryngoscopy
    • looking at the vocal cords with a microscope or endoscope typically  to look at or remove something from the vocal folds.
  • Cricothyroid approximation (CTA)
    • an operating room procedure to raise the lowest pitch you can produce. Primarily designed for transgender male-to-female patients.
  • Reduction laryngoplasty
    • an operating room procedure to reduce the height of your Adam’s apple. This is also known as a tracheal shave, though it is a procedure on the larynx (voice box) which connects to the top of the trachea (windpipe). Primarily designed for transgender male-to-female patients.
  • Feminization laryngoplasty
    • an newer operating room procedure (an alternative to CTA) to raise the lowest pitch you can produce by removing the front of the voice box and shortening the vocal cords. Primarily designed for transgender male-to-female patients.
  • Thyrohyoid elevation
    • a procedure to raise the resonance of the voice. Primarily designed for transgender male-to-female patients.

 

Excimer Laser

Laser that emits very concentrated light in the ultraviolet (UV) region of the spectrum.

Excimer lasers are used, for example, in:

Ophthalmology -- to vaporize part of the surface layer of the cornea and thus reshape the cornea to correct refractive errors from myopia(nearsightedness), hyperopia (farsightedness), and astigmatism.

An excimer laser uses a noble-gas halide. Excimer is a chemical term that refers to a substance formed by the joining of two atoms or two molecules of the same chemical composition in an excited state; it is an excited dimer.

 

Macular Degeneration Surgery

Dry macular degeneration is a chronic eye disease that causes vision loss in the center of your field of vision. Dry macular degeneration is marked by deterioration of the macula (MAK-u-luh), which is in the center of the retina. The layer of tissue on the inside back wall of your eyeball.

Dry macular degeneration is one of two types of age-related macular degeneration. The other type — wet macular degeneration — is characterized by blood vessels that grow under the retina in the back of the eye, leaking blood and fluid. Dry macular degeneration is the more common form of the disease.

Dry macular degeneration may worsen your quality of life by causing blurred central vision or a blind spot in your central vision. You need clear central vision for many tasks, such as reading, driving and recognizing faces.

 

Surgery to implant a telescopic lens in one eye

For selected people with advanced macular degeneration in both eyes, one option to improve vision may be surgery to implant a telescopic lens in one eye. The telescopic lens, which looks like a tiny plastic tube, is equipped with lenses that magnify your field of vision. The telescopic lens implant may improve both distance and close-up vision.

 

Cataract

A cataract is a clouding of the normally clear lens of your eye. For people who have cataracts, seeing through cloudy lenses is a bit like looking through a frosty or fogged-up window. Clouded vision caused by cataracts can make it more difficult to read, drive a car (especially at night) or see the expression on a friend's face.

Most cataracts develop slowly and don't disturb your eyesight early on. But with time, cataracts will eventually interfere with your vision. At first, stronger lighting and eyeglasses can help you deal with cataracts. But if impaired vision interferes with your usual activities, you might need cataract surgery. Fortunately, cataract surgery is generally a safe, effective procedure.

Cataract Surgery with Femtosecond Technology

Cataract surgery is one of the most frequently performed surgeries in the world today, enabling millions of people to regain eyesight lost because of cataracts.

Modern cataract surgery is safe, effective and highly predictable. And surgical tools and techniques are constantly evolving to make the procedure even better.

The next major advance in cataract surgery may be the use of femtosecond lasers — like those used in all-laser LASIK — to perform some of the steps in the cataract procedure.

The Laser Cataract Surgery Procedure

Using a laser for certain steps in cataract surgery has the potential to increase accuracy of the surgical procedure, decrease surgical time, decrease cataract surgery recovery time and improve visual results.

Basically, here are the four steps of modern cataract surgery and how these steps may be modified in laser cataract surgery:

  1. Creating corneal incisions. The first step of cataract surgery is to create an incision, usually in the periphery of the cornea, to enable the cataract surgeon to gain access to the cloudy lens inside the eye. Typically, these incisions are made with a hand-held surgical blade. In laser cataract surgery, they are created with a laser.
  2. Performing an anterior capsulotomy. The lens inside the eye is contained within a clear thin sack called the lens capsule. To break up and remove a lens clouded by a cataract, the surgeon must create an opening in the front part of the capsule (anterior capsulotomy). Typically, an anterior capsulotomy is performed with a hand-held cutting tool. In laser cataract surgery, this step is performed with a laser.
  3. Lens fragmentation. Prior to removal of a cataract, the cataract surgeon breaks the cloudy lens into smaller pieces, so it can be removed through the very small corneal incision. Typically, this is done with an ultrasonic probe. In laser cataract surgery, currently this step has not changed, but may change in the future.
  4. IOL implantation. After the cataract has been broken up and removed, the eye's natural lens is replaced with an intraocular lens (IOL), such as a multifocal IOL or an accommodating IOL to correct presbyopia. This step is not changed by laser cataract surgery.

And, if the patient has astigmatism, then an additional step may be incorporated into the cataract surgery procedure.

Creating limbal relaxing incisions (LRI). This step involves making a precise, partial-thickness radial incision in the peripheral cornea to alter the shape of the front of the eye and thereby correct astigmatism. Typically, a LRI is created with a hand-held, bladed tool (much like how RK was performed). In laser cataract surgery, this step may soon be performed with a laser.

 

Retinal Disease

The retina is a thin layer of tissue on the inside back wall of your eye, containing millions of light-sensitive cells and other nerve cells that receive and organize visual information. Your retina sends this information to the brain through your optic nerve, enabling you to see.

Retinal diseases can affect the area of the retina that serves your central vision (the macula and the fovea at the center of the macula). Many retinal diseases share common symptoms and treatments, but each has unique characteristics. The goal of retinal disease treatments is to stop or slow disease progression and preserve, improve or restore vision.

 

Rhinoplasty

Rhinoplasty is surgery to repair or reshape the nose.

Rhinoplasty can be performed under local or general anesthesia, depending on the exact procedure and the patient's preference. It is performed in a surgeon's office, a hospital, or an outpatient surgery center. Complex procedures may require a short hospital stay. The procedure usually takes 1 to 2 hours. It may take longer.

With local anesthesia, the nose and the area around it is numbed. You will probably be lightly sedated, but awake during the surgery (relaxed and not feeling pain). General anesthesia allows you to sleep through the operation.

The surgery is usually done through a cut (incision) made inside the nostrils. In some cases, the cut is made from outside, around the base of the nose. This type of cut is used to perform work on the tip of the nose or if you need a cartilage graft.

A splint (metal or plastic) may be placed on the outside of the nose. This helps maintain the new shape of the bone when the surgery is finished. Soft plastic splints or nasal packs also may be placed in the nostrils. This helps keep the dividing wall between the air passages (septum) stable.

Nose surgery is considered elective when it is done for cosmetic reasons. In these cases, the purpose is to change the shape of the nose to one that the patient finds more desirable. Many surgeons prefer to perform cosmetic nose surgery after the nasal bone has finished growing. This is around age 14 or 15 for girls and a bit later for boys.

 

Breast Augmentation

Breast augmentation — also known as augmentation mammoplasty — is a surgical procedure to increase breast size. During breast augmentation, breast implants are placed under the breast tissue or chest muscles.

For some women, breast augmentation is a way to enhance self-image and self-confidence. For others, breast augmentation is part of breast reconstruction after surgery for breast cancer or other conditions affecting the breast.

If you're considering breast augmentation, consult a plastic surgeon. Make sure you understand what surgery involves, including possible risks, complications and follow-up care.

Dental implant surgery is a procedure that replaces tooth roots with metal, screw-like posts and replaces damaged or missing teeth with artificial teeth that look and function much like real ones. Dental implant surgery can offer a welcome alternative to dentures or bridgework that doesn't fit well.

How dental implant surgery is performed depends on the type of implant and the condition of your jawbone. But all dental implant surgery occurs in stages and may involve several procedures. The major benefit of implants is solid support for your new teeth — a process that requires the bone to heal tightly around the implant.
