Zygomatic Implants and Full-Arch Fixed Teeth in Blumenau, Brazil

Clinica Dr. Fernando Giovanella is a private oral and maxillofacial surgery and implant dentistry clinic in Blumenau, Santa Catarina, southern Brazil, established in 2013. It is dedicated to one kind of patient: the person who has lost most or all of their teeth and has been told there is not enough bone for implants.

For these patients, zygomatic implants, the All-on-4 concept and graftless full-arch treatment can deliver a fixed arch of teeth within days of surgery, usually without bone grafting and without the year-long wait of the traditional path.

One patient at a time. The same surgeon, Dr. Fernando Giovanella, reads your scan, plans your case, operates, delivers your prosthesis and follows every review. There is no hand-off between professionals.

Is This Clinic Right for You?

You are probably a candidate if:

  • You were told you need a bone graft and a wait of 6 to 12 months before implants.
  • You wear a denture and want fixed teeth that do not come out.
  • You have implants that failed and need a revision or a complete rehabilitation.
  • You have advanced bone loss in the upper jaw and were told your case is too complex.

What the clinic does not offer: to keep its focus on complex full-arch cases, the clinic does not provide cosmetic dentistry (veneers, whitening, smile makeovers), orthodontics or aligners, root canal treatment, or routine dental care such as fillings and cleanings.

What Is a Zygomatic Implant?

Anatomical illustration: zygomatic implants anchored in the cheekbone supporting a fixed full-arch prosthesis

Without teeth, the jawbone loses stimulation and resorbs over the years, until a conventional implant has nowhere left to anchor. But one bone in the face stays dense and stable: the zygoma, or cheekbone.

A zygomatic implant is a longer implant designed to anchor in the zygomatic bone. Combined with the All-on-4 concept, it can support a full arch of fixed teeth in a single surgery, with no grafting. It is not experimental: it is an established technique, used for decades for severe bone loss in the upper jaw.

Zygomatic implants are used only in the upper jaw. In the lower jaw, the fixed full-arch prosthesis is placed on conventional implants.

Your Surgery Is Performed Three Times. Only the Last One Is on You.

The safety of a zygomatic surgery depends on knowing, precisely, where each implant goes before the surgery begins. Every case at the clinic follows the same three steps.

3D virtual planning of a zygomatic implant case on the computer

1. Simulation on the computer

Your CBCT scan becomes a 3D model, and the surgery is simulated: the position of each implant, millimeter by millimeter, already planned around the teeth that will sit on them.

3D-printed replica of the patient's upper jaw used to rehearse the surgery

2. A 3D-printed replica of your bone

The scan also becomes a physical replica of your bone. Dr. Giovanella holds it in his hands and checks everything defined on screen before the day of your surgery.

3. You, with the SkinGuide®

A custom guide printed in biocompatible resin transfers the simulated position to the surgery. The drill follows the planned path. The SkinGuide® method was developed by Dr. Giovanella.

From virtual planning to the final X-ray, and to the smile of a patient who can chew again.

FaceFirst: Planning Starts With Your Face

A fixed full-arch prosthesis has to be cleaned underneath, so it cannot have the pink rim (the flange) of a removable denture. But the flange is what supported the lip and cheek. Removing it can leave the face without support, which is a side effect few patients hear about.

Difference between a concave denture-like base and the convex, cleanable base of a fixed prosthesis

Left: a concave base, closed like a denture’s, which cannot be cleaned under a fixed prosthesis. Right: the convex base used at the clinic, which the brush and the water flosser can pass under.

The FaceFirst approach, developed by Dr. Giovanella, starts the plan from the support your lips and face need. Only then does it design a thin, convex, cleanable prosthesis, and only then are the implant positions defined. The approach was documented in a clinical case and published in a peer-reviewed journal:

Giovanella F. Graftless complete arch rehabilitation using a facial approach for enhanced bone anchorage and prosthetic outcomes: A clinical report. The Journal of Prosthetic Dentistry. 2025;134(2):295.e1–295.e6. DOI: 10.1016/j.prosdent.2025.03.046. PMID: 40221273.

Where Your Surgery Takes Place

The clinic is in the Centro Clínico Santa Catarina building, in the Garcia district of Blumenau. It has its own surgical suite for advanced dental surgery. Most cases are performed there under intravenous sedation given and monitored by a physician anesthesiologist dedicated to one patient, with no hospital stay.

Hospital Unimed Blumenau – Unidade Centro

Longer or medically complex surgeries, patients with relevant health conditions, and patients who prefer general anesthesia are treated at Hospital Unimed Blumenau – Unidade Centro, a day-surgery unit in downtown Blumenau, with same-day discharge.

Afraid of the dentist? You can be asleep.

Intravenous sedation is given by a partner team of physician anesthesiologists, with continuous monitoring. Many patients say they do not remember the procedure. Sedation is optional: many surgeries are done comfortably with local anesthesia, and the choice is made together at the assessment. Instrument sterilization is verified with biological indicator (spore) testing.

Coming From Abroad: How It Works

You find out whether you are a candidate before you book a single flight.

  1. Send your CBCT scan. A recent CBCT (cone-beam CT) scan of your jaws, as DICOM files, sent through PlacidWay. It is the only way to know if you are a candidate.
  2. Written reading of your scan. Dr. Giovanella reads the scan himself and tells you in writing which option fits you, or that you do not need zygomatic implants. It is preliminary until he examines you in person.
  3. Video consultation with the surgeon. A private video consultation with Dr. Giovanella himself, with a professional fee. He goes through your scan with you and answers your questions and your family's.
  4. Your written plan and quote. What will be done, how many days you need in Blumenau, and the cost, item by item.
  5. Your trip. Surgery and fixed teeth in the same stay: provisional teeth the same day, or the final ones 3 to 5 days after surgery, depending on your case. Plan about seven days in Blumenau. You come back once, three to four months later, for a planned review of at least three days.

Visa

Citizens of the United States, Canada and Australia need Brazil's eVisa, issued online only through the official site, brazil.vfsevisa.com. Apply at least four weeks before you fly. British, Irish and EU citizens need no visa for visits of up to 90 days.

Airport

Navegantes (NVT) is about 1 hour from Blumenau, with daily connections through São Paulo. Florianópolis (FLN) is about 2½ hours away.

Language

The video consultation, the consultations and the surgery are with Dr. Giovanella, in English. Day to day, the clinic's patient coordinator, Andreza, coordinates every step in English.

What you take home

An implant passport in English: every part placed, by maker, line, size and lot number, so any dentist at home knows exactly what is in your mouth. Implants: Neodent (Straumann Group) in most cases; S.I.N. in some zygomatic cases, chosen on your scan.

Indicative Prices

Price ranges in US dollars, valid from October 2026. They are a starting reference, not a quote.

Treatment Range (USD)
Full arch with zygomatic implants, fixed bridge, per arch 12,000 – 22,000
All-on-4, fixed bridge, per arch 6,000 – 10,000
All-on-6, fixed bridge, per arch 9,000 – 13,000
Single implant with crown 1,000
  • What changes the final price: the number and type of implants (zygomatic or conventional), one or both arches, the prosthesis material (acrylic, porcelain or zirconia), extractions or the removal of failed implants, and where the surgery is done (sedation at the clinic or general anesthesia at the hospital).
  • Your written quote, sent after the reading of your scan and the video consultation, lists each item: surgery, implants, anesthesia, provisional and definitive teeth, and reviews.
  • Not included: flights, accommodation and meals.

Frequently Asked Questions

"I have no bone at all." Am I still a candidate?

These are exactly the cases the clinic focuses on: the zygoma does not resorb like the jawbone. A CBCT scan confirms whether your case qualifies.

Is an implant in the cheekbone safe?

Zygomatic anchorage has been used for decades. At the clinic, every case is planned in 3D, rehearsed on a printed replica of your bone and guided by the SkinGuide®: the path is defined before surgery, not during it.

Does it hurt?

The surgery is done under sedation or anesthesia, so you do not feel the procedure. Afterwards, a medication and care protocol keeps discomfort under control; most patients report far less than they expected.

How long until I have fixed teeth?

Provisional teeth can be fixed the same day, or the final ones 3 to 5 days after surgery, depending on your case. The exact timeline is defined in your plan.

Do I need general anesthesia?

In most cases, no: intravenous sedation with a physician anesthesiologist, or oral sedation, in the clinic's surgical suite. General anesthesia at the hospital is an option for selected cases.

I have diabetes or high blood pressure. Can I have surgery?

Many medical conditions are manageable with proper assessment and preparation, including hospital facilities when indicated. Your medical history is reviewed before any travel is planned.

How is a fixed prosthesis cleaned?

Because the prosthesis is fixed and has a smooth, convex base, you clean it at home every day with a brush and a water flosser passed underneath and around the implants. At maintenance visits, the dentist unscrews it for a deeper cleaning.

Is the definitive prosthesis acrylic or porcelain?

Both exist, with different esthetics, wear, weight, repair and cost. The choice is individual and is made in your plan.

It Starts With Your Scan

Send your CBCT scan through PlacidWay. Dr. Giovanella reads it himself and replies in writing: whether you are a candidate, which option fits you, and what the next step is.
