- Ovarian Stimulation. This is accomplished with daily subcutaneous injections of hormones to stimulate the ovaries of the women. The drugs used are follicle stimulating hormone (FSH) and luteinizing hormone (LH). Ovarian stimulation lasts on average between 10 and 12 days. During this period the growth and development of follicles will be assessed using an ultrasound.
- Follicular aspiration. This procedure is intended to remove the eggs from inside the follicles. It is performed by an ovarian puncture with a needle inserted through the vagina and guided into the follicle using ultrasound visualization. This is an outpatient procedure that requires local anesthesia and sometimes depending on the patient and the location of the ovaries, the time required in the operating room to perform the procedure is 20-30 min.
- Fertilization. Is a process that begins with the entry of sperm to the egg. For fertilization to occur, about 25 000 to 50 000 previously trained sperm of the husband are incubated in the same medium capsule with each egg in the laboratory. The evidence of there being fertilization is given by microscopic visualization of the pronuclei (male and female), 16 to 20 hours after incubation of both gametes.
- Embryo transfer. It is an outpatient procedure that is performed in the operating room without analgesia or anesthesia. This procedure takes about 15 minutes and consists of depositing the embryos within the uterine cavity. It uses a thin plastic tube called inert and a very soft catheter. This is inserted through the cervix and once inside of the cavity, the embryos are placed in the uterine cavity. The number of embryos transferred depend upon the quality and age of the patient. So, if the embryos are transferred on the third day, they must continue to develop in the uterine fluid before making contact with the endometrium and begin implantation. In case there is an excess of embryos and these are of good quality they will proceed to their cryopreservation for use in a future cycle.
Intracytoplasmic Sperm Injection (ICSI)
- Male Factor (oligozoospermia, asthenozoospermia, and azoospermia teratozoospermia).
- Fertilization failure by conventional IVF.
- In Vitro Maturation.
- Use of eggs from Bank.
- Low number of eggs retrieved in egg retrieval.
- Presence of anti-sperm antibodies.
- Under the influence of any of these factors, it is necessary to facilitate fertilization by injecting a single sperm into the egg. This is done using microscopy equipment, equipped with systems that allow its introduction micromanipulation.
Assisted Hatching
- Eggs with thickened zona pellucida.
- Thawed embryo transfer.
- Women over 37 who have problems getting pregnant.
