Save a Child's Heart (SACH)
SACH [Save a Child’s Heart] is a community service initiative from the Apollo Hospitals with the aim of providing quality pediatric cardiac care and financial support to children from underprivileged sections of society suffering from heart diseases.

This is a need-based initiative, which covers all aspects of congenital heart disease, a major socio-economic problem in India.

Save A Child’s Heart was established in the year 2002 April with an aim of providing quality care and financial access to children belonging to lower socio-economic groups and in this short span of time has performed 900 surgeries/ interventions with a success rate of 97% and screened over 30,000 children with heart diseases across India

Save A Child’s Heart enjoys access to the most accomplished and distinguished cardiologists and cardiothoracic surgeons from Apollo hospitals who tirelessly work on transforming the lives of afflicted children by performing complex surgeries on them. The expenditure for the surgeries are undertaken by the Apollo hospitals along with support from philanthropic organizations and individuals .

The treatments include open and closed heart surgeries interventions, diagnostic procedures and follow up care for children suffering from heart diseases. The entire team of SACH including the specialists, nursing and paramedical staff volunteer their services. This team, made up of the most eminent pediatric cardiologists and cardiac surgeons from the Apollo group is one of the first and only team in India that performs the most rare and difficult surgeries.

Many complicated cases involving premature babies and infants are referred here. Even complex surgery techniques like Permembranous VSD, Device Closure, uni-focalization in children with VSD, Pulmonary atresia - single stage correction, Arterial switch operation in children with TGA     [Transposition of Great Arteries] and Valvoplasties in newborn are done. It is heartening to note that among the children operated by the foundation 50% would have died if timely medical interventions were not provided.

Why Save A Child’s Heart?

Pediatric Cardiology has been a neglected speciality in the country despite a massive disease burden. Consanguinity, viral infections like mumps, measles and rubella, absolute lack of awareness, and high treatment costs are some of the important factors contributing to the high disease burden.

It is estimated that over 200,000 children are born with heart diseases every year and less than 5% of them get optimal care on time. Most of these children succumb to the disease without celebrating even their first birthday.

The word "Cancer" is widely associated with impending death amongst a large number of people. This is understandable as Cancer is a killer disease. According to the W.H.O., globally one out of three women and one out of four men are likely to get cancer in their lifetime. With such a high probability rate, cancer can affect anyone, anytime and anywhere. However very few people know that cancer is curable in a large number of cases if detected early and a patient can lead a normal life. Although the prognosis of the disease is bad in an advanced stage, today the use of modern technology has brought the cure rate of cancer to almost 70-80%.

The Indraprastha Apollo Hospital, New Delhi has recently started the Apollo Cancer Institute, which is a comprehensive, multidisciplinary, state-of-the-art facility. It brings in the latest technology with the most competent and highly skilled health care professionals. This institute has the unique advantage of not being a stand-alone cancer unit but also having the most modern backup from all super specialties and diagnostics.

Electrophysiology
Electrophysiological studies have been used for decades to evaluate cardiac arrhythmias and to have basic understanding of their mechanisms. Insight helped the electrophysiologists to treat various arrhythmias and obtain permanent cure. Supraventricular tachycardia with accessory pathway or dual AV nodal pathway can be treated with over 90% success. Atrial flutter can be cured by ablation of isthmus between IVC and tricuspid valve with relative freedom from systemic embolization.

Indraprastha Apollo Hospitals has conducted two workshops with international luminaries who have demonstrated various technical details of electophysiological studies and Radio frequency ablations.

Venticular tacharrhythmias are the commonest cause of sudden cardiac death (SCD). It has been seen that more than 50 % patients are unable to reach hospital alive after myocardial infarction (heart attack) due to fatal ventricular tachycardias. Many patients with poor heart functioning (Ejection fraction < 40 % ) are at high risk of SCD. The patients are investigated by EP study and if required then provided with implantable cardioverter defibrillator (ICD) as a life saving device. Indraprastha Apollo Hospital has a tie up with passaic General Hospital, New Jersey and able to provide these expensive devices at much lower cost for Indian patients.

We have capabilities for multisite pacing for the patients of heart failure and Dilated Cardiomypathy. Recently a bilateral pacing was performed for intermittent Atrial Fibrillation.

Interventional Cadiology
The Cardiology Department is manned by senior Cardiologists trained in India and abroad. Coronary stenting is routine and performed by most of the invasive cardiologists. Patients with high risk and multi vessel involvement are also undertaken. Primary angioplasty, an approach of opening blocked coronary artery during acute myocardial infarction, in place of thrombolysis is practised in suitable cases. The cardiologists have performed over 15000 angiographies and nearly 3000 coronary angioplasties since the establishment of the department in January '96. Non-coronary intervention deals with variety of cardiovascular abnormalities. Subclavean, iliac, femoral and carotid artery balloon angioplasty and stenting are being performed. Vascular dissection and aneurysm repair are also conducted with covered stents.

Cardiac Treatment Facility

  • Non Surgical closure of Heart Defects
  • Patients with a hole in their heart, whether Atrial septal defect, small ventricular septal defect or

Patent ductus Arteriosus can be closed effectively by Button or Amplatzer's device or Coils. A young boy of four years having Patent ductus Atreriosus was taken for non surgical repair. Patent ductus was 4mm in diameter and was closed by coils by a modified arterial and venous approach. The abnormal communication was sealed off by placing three coils. The patient was discharged with no complications within three days. His progress is satisfactory after three years of this procedure.

Non-invasive Cardiology
The Department of Non-Invasive Cardiology is one of the busiest departments of the hospital and has state-of-the-art facilities. An average of 1400-1500 echoes are done monthly with a daily average of 50 echoes and total of 700-900 TMTs per month with an average of 25-30/ day. On a peak day there are as many as 50 TMTs & 75-80 echoes. The echoes include stress echoes, dobutamine stress echoes, vascular dopplers, transoesophageal echoes, intraoperative TEE and foetal echoes. The department constitutes of 2 divisions:
1. Catering to the indoor patients
2. Outpatient Officies of Doctors.

TThe Non-Invasive Cardiac Laboratory is run by Dr. P.K. Ghosh & Dr. K.K. Kapoor the two senior consultants, Cardiologists who are whole time dedicated to the work in the Non-Invasive Cardiac Laboratory. Besides them the staff includes Registrars & Jr.Consultants practicing non-invasive cardiology and technicians & stenos.

The indoor facility provides for echoes done in the echo room. Echoes are also done on an emergency basis round the clock indoors.

The out door department remains extemly busy with 25-30 echoes and a similar number of TMTs done daily and on a peak day these number could be 50 and 50 respectively. It may be pointed out that about 30-40% of the echoes are Vascular Dopplers and between 8-10 Stress & Dobutamine stress echoes are done daily. The policy is not to give appointments which makes the patient come back to the hospital again, but to do the echoes as they come. The waiting time is not more than half an hour and the department also gives typed reports within 15 minutes to half an hour of the procedure.

The department follows an active involvment in hands on training in echocardiography. These doctors come from all over India to learn and then subsequently go back and practice what they have learned.

Apollo Neurosciences Centre

Neurology
Neurologists treat disoders of the brain, spinal cord, nerves and muscles. Some common symptoms of neurologic disorders are:

  • Head ache
  • Speech &swallowing disorders
  • Fits
  • walking or balancing difficulty
  • Dizziness
  • Visual disturbances
  • Loss of conciousness
  • Tingling or numbness
  • Weekness & paralusis
  • Bladder control
  • Sleep disturbances

The department of Neurology is one of the finest in the country. Manned by half a dozen highly qualified full-time senior consultants, and backed by a state of the art clinical neurophysiology lab (EMG, NCV, EEG, Evoked Potentials), neuroradiology including computed tomography (CT), magnetic resonance imaging (MRI), digital subtraction angiography (DSA), extracranial and intracranial vascular Doppler, single photon emmission computed tomography (SPECT) and interventional neuroradiology, along with Asia's largest sleep lab (polysomnography) and excellent neuropathological backup, the department is equipped to manage any neurological disorder. World class critical care units(CCU) are geared to deal rapidly with 'brain attack' and other neurological emergencies round the clock. A physiotherapy and rehabilitation department offers exceptional neurorehabilitation to help stricken patients overcome their disabilities.

Clinical Neurophysiologic Services (Electrodiagnosis)

  • Electromyography (NCV/EMG)
  • Evoked potentials (EP)
  • Electroencephalography (EEG)
  • Sleep studies (polysomnography)

Electrodiagnostic studies can be helpful in evaluating weakness, numbness, pain and symptoms such as fatigue, cramps and abnormal sensation. Electrodiagnostic evaluation is an extension of the neurologist's physical examination and is performed by our clinical neuropysiologist who is a neurologist with special training in clinical neurophysiology. The time required to complete the study generally takes approximately 60 to 120 minutes. The two main procedures used to study nerves and muscles are needle electromyography (EMG) and nerve conduction velocity studies (NCV).

EMG
During an EMG, the neurologist analyses the electric activity in muscles by inserting a fine needle electrode into selected muscles. Needle insertion may cause mild temporary discomfort. The needle is not used for injection and no shocks are given. The physician can determine whether the muscle is working normally by seeing the electric activity on a screen and listening over a loudspeaker. The needles are discarded after use or sterilised to prevent the transmission of AIDS, hepatitis and other infections.

NCV
To perform nerve conduction studies, the physician tapes small metal electrodes on the skin and applies a brief electric stimulus to one portion of a nerve. Nerve stimulation will cause a tingling sensation. The physician can then evaluate the electric response of the nerve or muscle to which the nerve is attached and determine if the nerve impulse is a) conducted normally, b) at a slow speed or c) not transmitted at all, suggesting damage to the nerve.

Evoked Potentials
Electrodiagnosis may also include a number of other tests, such as evoked potentials. These studies use different stimuli, such as auditory clicks, a changing visual pattern such as a checkerboard, or small electric stimuli applied to specific nerves. The recordings are made over the surface of the head and the spine to evaluate whether the sensory impulses are conducting normally through the nerves, spinal cord or brain.

Special Precautions
The patient does not need to do anything special to prepare for this test, except to keep the skin free of any lotions or emollients on the day of the examination. Be sure to inform the physician, however, if you are taking blood-thinning medication such as Coumadin, have hemophilia or a cardiac pacemaker. Patients with myasthenia gravis should ask their physician whether or not to take anticholinesterase medications on the day of the test.

Patient Questions
This document is not a substitute for an informed discussion between a patient and his or her physician about the examination. If you have questions, they will be answered at the time of your examination. Copyright 1995 by the American Association of Electrodiagnostic Medicine. All rights reserved.

EEG
EEG is the single most important laboratory test in the evaluation of patients with seizures and related disorders. It is a painless procedure, during which the brain's electrical activity detected by electrodes pasted to the scalp is amplified and recorded. The test lasts for approximately one hour. Patients are advised to keep their hair free of any oil and grease and not to be fasting before the test. They should also continue taking any medicines prescribed to them. At Apollo Hospitals, advanced computerised Digital EEGs offer off-line data analysis for greater diagnostic yield.

Neurosurgery
"The tools used by the surgeons must be adapted to the task and where the human brain is concerned they cannot be too refined."
by Lars Leksell

History
Neurosurgery has undergone a sea change since the days of Harvey Cushing. The development of bipolar cautery, the use of the surgical microscope, the availability of the ultrasonic surgical aspirator have all contributed to decreasing morbidity and mortality. Advances in neuroanaesthesia, neurosurgical intensive care and neuro-imaging technology have played a significant role. We are now in the age of Minimally Invasive Neurosurgery. The goal and the aims of treatment have shifted. It is no longer enough to save life but preserving of all functions, cosmesis and reducing pain, shorter hospital stay, avoiding discomfort are equally important. To fulfill these aims of excellence in neurological health care, the Apollo Institute of Neurosciences has been set up as a part of Dr. Reddy's vision of World Class Health Care facilities in India.

The Apollo Institute of Neurosciences is equipped with facilities at par with the best in the world. The most modern technology and equipment is provided to cover the entire range of neurological diseases. About 1000 major neurosurgical operations are being carried out every year. Transsphenoidal surgery for pituitary tumor, spinal fusions for congenital and traumatic (CT scan & MRI for locating the target for functional neurosurgery) and intra-operative monitoring for different kinds of surgery are all being done on a routine basis.

The highlights of the facilities available are :

Dedicated Neurosurgical and Neuro Anaesthesia Team
The department consists of five full time, one part time and several visiting Senior Consultant Neurosurgeons, each with their own associate consultants. There are four dedicated neuroanaesthesiologists to run the anaesthesia services. Two operating theatres with nursing staff and eleven bedded dedicated neurosurgical intensive care unit. Intra-operative electro physiological monitoring for spinal surgery, functional surgery and posterior fossa surgery is being carried out by an expert specially trained for intra-operative monitoring.

Diagnostics
The Diagnostics department is fully complete with facilities for Computerised Tomography (CT), Magnetic Resonance Imaging (MRI) and Angiography. All routine X-rays and ultrasound investigations are carried out here. The department of Radiology consists of thirteen Senior consultants, Associate consultants, Junior Consultants, Registrars and Sonologists.

Operating Rooms
The two neurosurgical theatres are ultra modern and equipped for all types of neurosurgical operations. Both theatres have a laminar air flow system and are equipped with the latest ceiling mounted microscope with in-built still photography and video recording facilities. One of them also is capable with the optical tracking system (OTS). OTS allows the monitoring of the progress of surgery in real time. Residual tumors, haemorrhage etc. can all be visulised.

The Intensive Care Unit
The modern eleven bedded facility is equipped with the latest Modular invasive monitoring system (HP) and advanced life support equipment capable of managing complicated neurosurgical and neurological patients effectively. The nursing ratio is one to one. All associated specialists like respiratory physicians, nephrologists and endocrinologists are available round the clock.

Specialised Equipments

Dedicated X-Knife
An exclusive department in the hospital houses this machine which is the first dedicated X-Knife outside the United States. X-knife has the added advantage of being used for fractioned treatment (stereotactic Radiotherapy-SRT) for benign and malignant tumors not suitable for SRS.

Computer Assisted Neurosurgery
The Apollo Hospital has now taken the first step in this part of the world with the availability of this new technology. This was the first centre in North East Asia to have this equipment. Surgeons at the Apollo Institute of Neurosciences are now using an operating computer with an accuracy of 1mm in which images of the patient's brain can be directly transferred from the MRI or CT Scan to the Operating Theatres at the Operating Table. The surgeon can join the probe to different parts of the brain and retrieve the images previously taken. This is of great help not only in locating difficult tumors, but also in avoiding critical structures and ensuring completeness of excision. Surgery for abnormal movements, epilepsy and small deep malformation is being done.

Frame Based Stereotaxy
The Radionics CRW System in this hospital is complete with full range of accessories to include the ability to carry out Frame Based Stereotactic Surgery including Craniotomies. Equipment is also available to generate RF lesions and has been used successfully for functional neurosurgery procedures like pallidotomy and thalamotomy for treatment of movement disorders and parkinsons disease. Image guided Frameless stereotaxy allows safe surgery in eloquent areas. This Apollo Institute of Neurosciences is one of the first hospital to have this kind of complete technology in this part of the world.

Interventional Neuro Radiology
A separate Department and a separate machine is available for this crucial area. The development of such a centre was a priority with this hospital. This machine has biplane rotational angio facilities and is being utilized for embolization of arteriovenous malformation, treatment of giant aneurysms by proximal occlusion and endovascular packing using GDC system. Thrombolysis by injection of Urokinase directly into thrombus in the specific artery in the treatment of brain attack is being done. Angioplasty and stenting is being done to prevent stroke. Vascular lesions such as dural AV malformations, carotid-cavernous fistula hemangiomas of the head and neck, vascular tumors are all being treated. More than 150 therapeutic procedures and more than 300 diagnostic procedures are being done annually.

Microsurgical Instrumentation
Two complete sets of Microsurgical Instruments are available for all surgeons to carry out full range of microsurgery for cranial as well as spinal surgery.

Neuroendoscopy
Equipment for minimally invasive endoscopic removal of certain tumors (eg. intraventricular and pituitary tumors), third ventriculostomy in the treatment of hydrocephalus, drainage of brain abscesses and cysts is used routinely.

Spinal Instrumentation
A complete range of spinal instrumentation is available for stablising the spine using metal implants. These are used in cases of spinal instability from spinal injuries, Tuberculosis & tumors. This allows early ambulation, facilitates rehabilitation and early discharge from hospital.

Fetal Medicine
Fetal Viability Scan 6-10 weeks

This is an ultrasound examination that is usually carried out vaginally. It aims to determine the number of fetuses present and whether the pregnancy is progressing normally inside the uterus. This scan is useful for women who are experiencing pain or bleeding in the pregnancy and those who have had previous miscarriages.

First Trimester Screening
We are only unit in north India who are accredited to perform the First Trimester Down’s Syndrome Screening according to the standards laid down by the Fetal Medicine Foundation UK. This screening involves an ultrasound assessment of the fetus including the nuchal translucency, and the new markers such as tricuspid regurgitation, ductus venosies, nasal bone and facial angle assessment.

Anomaly Scan 18-23 weeks
This is a detailed scan during which each part of the fetal anatomy is examined. Special attention is paid to the brain, face, spine, heart, stomach, bowel, kidneys and limbs.

If any abnormalities are detected the significance of the findings will be discussed and the couple will be given the opportunity to have further counseling with a Senior Fetal Medicine Consultant

Cervical Screening 22-24 weeks
This is a transvaginal scan done to assess the women risk of going into preterm labour and delivery at less than 33 weeks by measuring the cervical length.

The uterine artery Dopplers are also checked to assess the woman’s risk of developing Pre-eclampsia or intra-uterine growth restriction.

Fetal Echocardiography
A detailed examination of the fetal heart and connecting vessels is carried out usually at 20 weeks by a consultant cardiologist. It is especially recommended for people with a family history of heart abnormalities or where increased nuchal translucency has been found at the 12 week scan. It is also an important ultrasound scan when other abnormalities have been found in the fetus.

Fetal Well Being Scans 24-42 weeks
Some obstetricians advise that an ultrasound scan to assess fetal well being is offered to all women at about 30-32 week of pregnancy. Others reserve such scans for those women who have had previous complications of pregnancy such as pre-eclampsia, growth retardation, diabetics, stillbirth and for those women who develop an abnormality during the course of their current pregnancy.

Invasive Procedures
Chorionic villous sampling

CVS is a medical procedure performed after 11 weeks of gestation in which tiny pieces of the chorionic villi, the early placenta (a tissue on the outside of the fetal gestational sac) is obtained for chromosome analysis.

Amniocentesis
Amniocentesis is a medical procedure in which a small amount of amniotic fluid is with- drawn from the amniotic sac surrounding the fetus. The amniotic fluid is then processed to separate the fetal cells for further genetic testing.

Fetal blood sampling
Fetal blood sampling as part of diagnosing, treating, and monitoring fetal problems at various times during pregnancy. A fetal blood sample may be taken to: diagnose genetic or chromo-some abnormalities check for and treat severe fetal anemia or other blood problems such as Rh disease check for fetal infection give certain medications to the fetus.

There are several ways blood can be obtained from the fetus. After cleansing the mother’s abdomen with antiseptic, a long, thin needle is inserted into the mother’s uterus guided by ultrasound. Blood may be taken from the following sources: blood vessels of the umbilical cord (also called cordocentesis, or percutaneous umbilical blood sampling, or PUBS) a fetal blood vessel, usually in the liver or heart.

Fetal blood transfusions are performed using a similar technique in cases of fetal anaemia.

It is also useful in the management of wide range of critical or even conventionally incurable aliment.

Nature-Cure
Nature-cure , as enunicated by Ayurveda is based on the principle that health is a state of dynamic balance of the human individual with the five elements, viz Prithvi (erath), Apa (water), Tejas (fire), Vayu (air), and akasha(space). This harmony can be achived by a disciplined lifestyle, diet modification, message, sun bath, stem, aerobics, yoga and accpuncture. etc.

Panchakarma
The practice of snehna (massage with herbal oils) and swadhana (steam bath) restore a balance of the external (5 elements) and internal (emotional) factors in the body, thus promoting inherent healing.

Yoga
Yoga as a way of life helps to attain mental and emotional well being, as well as vigour and physical fitness. Regular practices of Pranayama, Meditation and Asanas combats stress and restores wholesome harmony.

Pranayama
Pranayama is the artof breathing. Breath is the vehicle of vital life force, the 'prana' that connects the human microcosm with and healing.

Meditation
Meditation is the state of heightened awareness whereby we get connected to our inner conscious. Its practice makes our mind deeply relaxed necessary to overcome stress and fatigue, and restore homeostasis
" Samah Bhutva Samatvm Yogah"
-equilibrium is yoga
- Bhagvad Gita

Acupuncture
The practice of acupuncture has been developed from traditional Chinese Medicine. In this, the dynamic life force 'Chi' views the human organism as one macrocomic unit of the macrocosmic whole, inter-linked through strategic points along the acupuncture meridians on human surface. The 'Chi' is regulated to restore harmony in Yin-Yang energy, to achieve homoeostasis. This activates the individual's intrinsic natural healing to restore health.

The traditional acupuncture require insertion of hair-fine sterile needles at acupuncture points. While the use of needles is absolutely safe and almost pain free, several non-invasive modalities to complement/ substitute needles are also frequently used.

The modern revival of acupuncture is due to its recognition by the World Health Orgnisation (WHO), the National Institute of Health (NIH) & food and Drug Administration (FDA) in USA and National Health Service (NHS) in UK. It is also free from side effect and cure most of the diseases in well experienced hands.

Reflexology or acupressure is an ancient oriental method that restores health and well being by correcting the disturbed flow of vital 'life force' through the human body.

Pranic Healing and Reiki are also holistic techniques that heal the body and mind simultaneously, wherein healing is achieved by removing diseased energies from the patient's invisible bioplasmic energy body and replacing it with fresh vital energy called the 'parna' or the 'ki'.

Modern day living involves tremendous mental stress leading to various psychosomatic ailments. The best way to combat this is to get an opportunity to relax and rejuvenate through Holistic healthcare.

Hyperbaric Oxygen Therapy
Hyperbaric oxygen therapy (HBOT) is a medical treatment that uses the administration of 100 percent oxygen at a controlled pressure (greater than sea level) for a prescribed amount of time-usually 60 to 90 minutes. It helps the body heal itself by making oxygen available to body tissues, or organs, which are not receiving an adequate supply because of illness or trauma.

How does HBOT work and why is it effective?
The air we normally breathe contains about 21% oxygen, which is carried throughout our bodies by red blood cells in blood. During HBOT, the 100% oxygen we breathe, combined with higher pressure, delivers up to 15 times the oxygen content to all body fluids, including plasma, cerebrospinal fluid surrounding the brain and spinal cord, lymph, and intracellular fluids. This allows oxygen levels to increase even in areas with blocked or compromised blood supply as a result of injury or illness, as well as in areas of tissue damage.

Increasing tissue oxygen levels produces several important long term therapeutic benefits including enhanced growth of new blood vessels; increased ability of white blood cells to destroy bacteria and remove toxins; increased growth of fibroblasts (cells involved in wound healing); and enhanced metabolic activity of previously marginally functioning cells including brain neurons.

Administration of HBOT
Hyperbaric Oxygen Therapy is given in a special chamber which is pressurized with compressed air. At a certain controlled pressure patients breathe 100% oxygen through a mask, hood or endotrachial tube. 4-6 patients can be treated at the same time. A staff nurse is present at all the times with patients inside the chamber while the treatment is monitored from a Control Panel outside the chamber.

Indications of HBOT

Hyperbaric Oxygen Therapy unit is managed by one of our senior consultants. Hyperbaric oxygen therapy is a form of medical treatment in which patient breath 100% oxygen at a pressure greater than the atmospheric pressure. Patient is entirely enclosed in pressure chamber & breaths 100% oxygen at a pressure greater than one atmospheric absolute (ATA). These treatment chambers can deliver oxygen concentrations to tissue at levels higher than ant existing oxygen delivery methods. ATA is the unit of pressure and one ATA is equal to 760 mm of mercury of pressure at sea level.

Pre-operation checks :
Senior consultant of department of internal medicine are involved in preoperative checks in addition to anaesthetists assessment to manage &pick up existing medical problem prior to surgical procedures as well as to take appropriate measures prior to surgeries. Often consultants are required to manage medical problems coexisting with surgical conditions. Infertility Treatment Services At “AARU”
Introduction:

In all branches of medicine both patients and doctors recognize that same symptoms may have many different causes and that accurate diagnosis should always come before treatment. This is no less true when the symptom is the “inability of a couple to have a baby (infertility)”. Therefore before you start infertility treatment we recommend you to have proper examination and investigations so that we can give you better advice regarding the treatment options and help you to decide the most appropriate treatment for you.

Assisted Reproductive Technologies (ART):
Any medical technique that attempts to obtain a pregnancy by means other than by intercourse is defined as ART. These techniques involve the manipulation of sperm or Oocyte or both, and the gametes or the embryos are transferred into the uterus or fallopian tubes. ART includes Intrauterine Insemination (IUI), In Vitro Fertilisation-Embryo Transfer (IVF-ET), Intra Cytoplasmic Sperm Injection (ICSI), Assisted Hatching (AH) etc. We at ARRU provide you the full range of ART services to aid your infertility treatment to help you bear a child. Successful outcome with minimum invasion is always our priority.

Intra Uterine Insemination (IUI):
This is the method by which processed semen is placed directly in the uterus with the help of a catheter. IUI can be done in a natural cycle, however, to get better results, it is usually preceded by ovulation induction / ovarian stimulation using appropriate fertility enhancing drugs. With a policy of initial treatment by IUI approximately 20 percent couples (cumulative pregnancy chances) would achieve pregnancy. However, there is not much benefit by continuing same treatment repeatedly and after 3-4 unsuccessful cycles of IUI, couple should consider IVF or ICSI treatment.
Advanced female age, poor post wash semen quality and a history of tubal factor or previous pelvic surgery are significant risk factors for poor IUI success rates.

In Vitro fertilisation (IVF) and embryo transfer (ET)
Commonly known as "Test Tube Baby". IVF means fertilisation of an ovum outside the body and consequently transfer of the fertilised ovum (embryo) into the uterus of the woman. IVF is probably the most widely practised assisted conception procedure in the world.

The procedure does not need admission at any step and is conducted on outpatient basis.

Steps involved in IVF procedure:

  • Ovarian stimulation by hormonal injections to produce multiple eggs.
  • Monitoring of the response by ultrasound scans and blood tests.
  • Egg retrieval with the help of a needle under local / general anaesthesia.
  • Fertilisation of the eggs in the laboratory.
  • Transfer of the resulting embryo(s) into the uterus of the woman.
  • Blood test performed 15 days after embryo transfer to access the establishment of pregnancy.

If the treatment procedure is successful, one or more embryos will implant in the uterus and the pregnancy will result, just as it happens in the natural process of conception.

Indications for IVF:
IVF is helpful in a variety of infertility cases involving :-

  • Bilateral tubal blockage or abnormality.
  • Male factor infertility.
  • Unexplained infertility.
  • Where ovulation induction/stimulation (OI / OS) & IUI has failed.
  • Endometriosis.

Intra Cytoplasmic Sperm Injection (ICSI ):
ICSI is the technological breakthrough in the field of IVF which is used to overcome the inability of sperm to fertilise an egg which may be either due to sub-optimal sperm parameters, Oocyte defects or other reasons (anti sperm antibodies). In this technique a single sperm is directly injected into the cytoplasm of an egg in order to achieve fertilisation.

Indications for ICSI:

  • Oligospermia - very low sperm count in the ejaculate.
  • Asthenospermia - motility less than 40 %, in the ejaculate.
  • Teratozoospermia - < 14 % normal forms, according to strict criteria.
  • Necrozoospermia - all dead sperm in the ejaculate.
  • Antisperm antibodies.
  • Repeated fertilisation failure after conventional IVF-ET.
  • Ejaculatory disorders.
  • CBAV - congenital bilateral absence of the vas deferensObstruction of both ejaculatory ducts.
  • Failed vasectomy reversal.

The steps involved in ICSI procedure are exactly the same as for IVF, except that fertilisation is achieved with the help of a micromanipulator.

In some cases when there is a complete absence of sperm in the ejaculate, sperm can be retrieved from the testis / epididymes and used for ICSI.
The testicular / epididymal sperm retrieval techniques are:

  • PESA - Percutaneous Epididymal Sperm Aspiration.
  • MESA - Microsurgical Epididymal Sperm Aspiration.
  • TESA - Testicular Sperm Aspiration.
  • TESE - Testicular Sperm Extraction.

IVF Using Donor Oocyte (Eggs):
Women who are unable to produce their own eggs or their eggs failed to fertilise during an IVF cycle due to poor Oocyte quality can be helped by using other women’s egg (Donor Oocyte). Replacement of donor embryos and surrogacy are other available options to the infertile couple.

Assisted Hatching ( AH ):
The procedure is based on the fact that an alteration in zona pellucida (outer covering of egg) either by drilling a hole through it or by thinning it, will promote hatching or implantation of embryos that are otherwise unable to escape intact from the zona pellucida.

Cryo Preservation of Embryos ( embryo freezing ):
Excess (Surplus) embryos can be Cryo-preserved at ultra low temperatures for many years. These frozen embryos can be used subsequently without the need for ovarian stimulation and egg retrieval. However the pregnancy rate following transfer of frozen embryos is lower than that with fresh embryos.

Semen /Sperm freezing:
Semen /Sperm can be stored frozen at ultra low temperatures for long duration which could be helpful in a variety of circumstances such as:

  • Semen freezing prior to chemotherapy.
  • Inability of the male partner to be present or to deliver the semen on the day of procedure(however, the frozen semen is used as a standby and fresh sample is always preferable)
  • Testicular biopsy / sperm freezing to avoid repeated biopsies.
  • Donor semen, until getting a repeated investigation done for the infectious diseases.

Semen Bank:
We are having a semen bank at ARRU. The bank contains frozen sperm from voluntary donors with various backgrounds and physical characteristics to match with the individual needs. Donors are screened thoroughly to rule out transmission of any infectious or genetic diseases.

Investigations Required For An Art Procedure:

For successful conduction of the ART procedure(s) the couple has to undergo certain investigations:

  • Investigations for female partner:
    1. General health check tests.
    2. Hormonal assay.
    3. Invasive tests- HSG, Laparoscopy, Hysteroscopy
  • Investigations for male partner:
    1. Semen analysis
    2. Semen culture & sensitivity
  • Investigations for both the partners:
    1. HIV
    2. Hepatitis B
    3. VDRL
    4. Blood group

Risks involved:

Multiple pregnancy: To enhance the chances of success (approximately 10% with single, 20% with two, 30% with three ), more than one embryo are usually transferred at a time, in some cases all the transferred embryos get implanted leading to multiple pregnancy. Most people would not mind twins but conception with three or more embryos/fetus would be a serious set back. You would be given the choice regarding the number of embryos to be transferred, however if you conceive with multiple fetuses a minor surgical procedure under USG guidance can be carried out on an outpatient basis to reduce the number of foetuses conceived ( selective foetal reduction).

Ovarian Hyperstimulation Syndrome (OHSS): OHSS is a potential complication of ovarian stimulation therapy, patho-physiology of which is not clearly understood. The risk of OHSS is somewhat directly related to the number of follicles and the hormone levels. Therefore sometimes it may be advisable to cancel the cycle before egg retrieval if risk of OHSS is extremely high. However even with only a few follicle it is not impossible for the patient to experience OHSS problem. Mild cases of OHSS may pass unnoticed while severe cases may lead to abdominal distension, vomiting, breathing difficulty, reduced urinary output etc. the incidence of OHSS varies from 8-23% in reported literature most of these cases have mild to moderate problem but 1-7% of patients may require hospitalisation and supportive treatment. If no pregnancy occurs this syndrome will typically resolve within 1-2 weeks, in the setting of a maintained pregnancy slow resolution of symptoms usually occurs and may take up to 1-2 months.

Failure at different steps of IVF treatment: Despite the best attempt to stimulate ovaries in few cases ( advanced age, raised FSH levels etc.), there may be no follicular development or no oocytes may be obtained from the follicles following oocyte retrieval procedure, or all oocytes may fail to fertilize due to oocyte or sperm factor. In such cases there will not be any embryos for transfer. These patients may be advised to consider donor Oocytes, sperm or embryos in their future attempts.

Ectopic pregnancy: The ectopic pregnancy rate following IVF pregnancies varies from 2-11 % as compared to 2.5% in general population. The reason of ectopic pregnancy after IVF and embryo transfer is multifactorial.

The incidence of congenital malformation in IVF babies is no more than in the general population. However following ICSI whether there may be a very slight increased incidence of chromosomal (especially the sex chromosomal) aberrations which may have been the original cause of infertility, has not been completely resolved.

Success rate:
IVF has a variable success rate in different patients. The reason for this does not lie in the technique itself, but is due to the variety of abnormalities which are causing the basic infertility. Some of these abnormalities respond well to IVF while others do not. This means that some patient will have a good chance of pregnancy others will have a poor chance.Success rate of a center represent the number of pregnancies achieved out of total number of cases performed over a period of time. At our center we are currently having 40-50 % pregnancy rate per attempt.

Surgery of the liver is very complex and specialized procedure that is best undertaken by trained experts in this field as in Apollo. Complex liver surgeries can be undertaken by the liver transplant team.

Liver failure treatment and Liver Transplant Service

What is liver failure? What causes it?
This is a potentially fatal condition in which the liver progressively stops performing its function resulting in episodes of jaundice, accumulation of fluid in the body, blood vomiting and confusion. Conditions such as Hepatitis B or Hepatitis C infection, excess alcohol consumption, diseases such as Primary Biliary Cirrhosis and Primary Sclerosing Cholangitis can cause this state.

Can it be treated?
With modern medicine, it is now possible to treat most such cases successfully. While a proportion of patients with acute liver failure can recover by receiving treatment in specialised liver ICUs such as the one available at Apollo, most other patients ultimately require liver transplantation. Medicines, and other measures such as draining ascites and stopping the blood vomiting by endoscopy are only temporary and do not halt the progress of the disease. Most such patients die within a few months to a year unless liver transplantation can be performed.

Is such treatment available in India?
Liver transplantation is an accepted treatment for liver failure in the west. Abroad, the Indian patient goes on a low priority lwaiting list and is considered for transplantation only when a donor organ has been found unsuitable for all the native patients. Secondly, the minimum expense for a liver transplant in the USA for example, is around $ 350,000 hence only a handful have been able to successfully receive transplants abroad. Now, Indraprastha Apollo Hospitals has the state-of-the-art facilities for managing adult patients and children sufferring from liver failure. Liver transplants are being performed here at fraction of the cost of having it done in the west

Our liver specialists/ transplant experts have the distinction of having trained and worked for several years at the worlds' most renowned centres in USA, U.K. and Japan where they have successfully treated hundreds of such patients with expert ICU management and liver transplant surgery. They are well-versed with the transplantation techniques in adult and children using either organs from brain dead donors or portion of liver from a living related donor. The liver transplant unit is the only one of its kind in whole of south and west Asia to carry out successful adult- adult living donor right to be liver transplantation. More than 20 Liver transplantation have been carried out till date. The department at Apollo boasts of a dedicated 5 bedded Liver ICU manned by trained doctors and nursing staff and fully equipped with state-of-the-art ventilators and monitors, high-tech operating rooms with anaethesia staff trained with liver transplant anaesthesia and round-the-clock investigation facilities including an advanced Transplant Immunology lab support and cyclosporin estimation.
